Job Summary:

The Non-Medical Case Manager (NMCM) provides guidance and assistance to HIV positive clients in accessing medical, social, community, legal, financial, and other needed community services. The NMCM is responsible for ensuring services are provided from the initial encounter with the client throughout the continuum of care, and for submission of timely reporting to grantors. Responsibilities also include performing program intake for new clients and client follow up.

Essential Functions & Responsibilities:

  • Obtains and/or verifies all relevant client demographic and insurance information to determine eligibility for the Ryan White program.

  • Completes applications, as well as semiannual and annual certifications, to ensure no lapse in eligibility or loss of benefits occurs.

  • Presents clients with appropriate paperwork based upon Ryan White standards for eligibility.

  • Reviews client accounts for outstanding balances.

  • Reviews eligibility status for monthly invoices that are submitted to MWHC and HRSA/HAHSTA/NVRC, with the guideline of Ryan White being the payer of last resort.

  • Ensures all services are authorized and documented in the EHR for invoicing.

  • Prepares monthly requests for reimbursement and interfaces with the Finance department to facilitate submission of all information.

  • Completes all Ryan White client and program reimbursement billing processes on a monthly basis and submits to appropriate reporting area.

  • Receives, resolves, and prepares all incoming invoices for approval for services provided to clients.

  • Assists clients with accessing benefits programs such as Medicaid, Social Security Insurance (SSI), Social Security Disability Insurance (SSDI), Medicare, Temporary Assistance for Needy Families (TANF), and Supplemental Nutrition Assistance Program (SNAP); as well as other services in the community.

  • Links clients to Ryan White services such as: prescription drugs (co-pay assistance and deductibles), ADAP, housing, food bank and delivered meals, oral health care, vision, mental health and counseling, case management, peer mentor, transportation services, psychosocial support, and the Affordable Care Act (ACA) program.

  • Prepares referral and client financial assistance documentation for clients and maintains records in their electronic health record.

  • Provides advice and assistance to clients in obtaining medical, social, community, legal, financial, and other needed services.

  • Utilizes CareWare client data system to enter and scan client service records.

  • Uploads eligibility documents into electronic records system within three days of eligibility.

  • Maintains meeting minutes and attends meetings required by Ryan White staff.

  • Performs other duties as assigned.

Qualifications:

  • High School diploma or equivalent required; Associates degree preferred.

  • Minimum of one year of experience in a healthcare setting required.

  • Financial analysis experience required.

  • Experience with Microsoft Word and Excel required.

  • Experience in customer service required.

  • Experience working with grants preferred.
