Reporting to the Referral/Intake Manager. Working in an assigned territory or facility, facilitates referrals to agency by gathering and interpreting medical and financial data, meeting with patients and families and working with case managers to ensure optimal transition from hospital or other facilities to home. Requirements: Graduate of accredited RN program 3-5 years directly related#experience RN license

  • NH or other Compact State Drivers License

  • Valid/Satisfactory Driving Record CPR certified or within 30 days of hire Major Responsibilities: Interprets policies/ regulations and acts as resource for case managers, physicians, hospital and community for effective public relations and marketing of services.Successfully builds relationships with case managers, social workers etc. and may assist with contracting process. Demonstrates the ability to tailor plan of care based on unique requirements of the patient#s age and developmental needs. Coordinates care plan, suggesting appropriate services to family and staff and communicates changes as needed. Initiates referralâ€'toâ€'admission process for each patient referred by facilitating required data, leveraging available resources to ensure data is complete and accurate. Provides accurate and thorough documentation to Intake for additional data entry. Provides accurate and thorough documentation on medical record and collects appropriate signed forms. Conducts individual and group presentations on products and services in an informative, accurate, and professional#manner. Responds to questions about products and services. #Facilitates communication between Business Development and Hospital, Skilled Nursing Facility representatives, and#other referral sources. Relates opportunities for recovery activities. Keeps agency informed of facility changes regarding bed capacity, staff changes, new facility programs and/or procedures. Participates as needed in relationship building opportunities within liaison sites and RVNA#H service communities. Demonstrates the ability to perform all assigned duties related to intake and Liaison while fostering an environment of#teamwork and patient advocacy with both internal and external relationships. Performs duties as assigned.
