What You Will Do

As a Patient Navigator in the Oncology department you will be working closely with the Chief of Oncology. Your focus is to educate and support Lung Cancer patients and their families. Lung Cancer is the #1 initative for Saint Thomas Oncology. This is a highly visible role. The Patient Navigator will meet with patients in the clinic alongside doctors. You will also plan, coordinate, set up, and attend 2 conferences weekly with medical team. The Patient Navigator must have a passion for Oncology patients and their families.


  • Functions as a liaison for management staff, administration, physicians, managed care companies, community organizations, patients, and other customers.

  • Coordinates efforts for treatment and promotes quality improvement and educational efforts.

  • Compiles data, tracks outcomes, and makes recommendations for process improvement.

  • Provides referrals/support for follow-up care for patients who are uninsured or underinsured.

What You Will Need


  • Required Credential(s):

  • Licensed Registered Nurse credentialed from the Tennessee Board of Nursing or current home state license for multi-state license recognition "Compact State" obtained prior to hire date or job transfer date.

  • Preferred Credential(s):

  • BLS Provider credentialed from the American Heart Association (AHA).

  • OCN is a plus


  • Diploma

  • Diploma from an accredited school/college of nursing required

Preferred Experience:

  • At least 1 year of experience in a Patient Navigator, Nurse Navigator, or Survivorship Navigator role

  • Oncology background is preffered

Aspire's clinicians (e.g., physicians, nurse practitioners, nurses and social workers) are experts in providing patients with relief from the symptoms, pain, and stress of a serious illness; helping patients and caregivers navigate the healthcare system; guiding patients and caregivers through difficult and complex treatment choices; and providing emotional and spiritual support to patients and their families. Aspire currently operates in multiple markets and is in the process of building a team of professionals to implement its innovative palliative care program across the country. Aspire Health is seeking Triage RN to support a team of clinicians in delivering comprehensive palliative care services. Primarily, this individual will be based in Nashville and will be responsible for providing all telephonic and back-office support required by the team of clinicians. This will include a range of clinical and administrative tasks such as directly providing telephonic support to Aspire's patients, families, and Aspire's team of community-based Nurse practitioners and Social Workers. Ideal candidates will have experience identifying and coordinating the needs of chronically ill patients and families as well as supporting clinicians to help them operate as efficiently as possible. Responsibilities will be highly varied, and appropriate candidates will be multitalented, flexible problem solvers who are eager to tackle complex problems and tasks. * Telephonic care coordination with palliative care patients and their families, including triaging incoming calls as well as making proactive outgoing calls as requested by Aspire's Nurse Practitioners * Assess, identify and close clinical and non-clinical gaps in patient care * Appropriate triage and escalation of routine, urgent and emergent patient calls * Clinical care coordination with PCP's and other providers and specialists, including * Aspire NP home visit coordination * PCP or specialist office visit appointment coordination * Initiating clinical service orders on behalf of the patient * Prescription updates and refill requests * Aspire field staff referrals * Initiating health plan authorization for Home Health, DME, Home Infusion and other critical services. Verifying the timely delivery of these services. * Medication management, including identifying potential medication issues, answering patient questions on side effects and medication conflicts, and coordinating refills. * Identifying community resources and assisting Aspire's patients and families in accessing those resources * Referral to Aspire Health social worker and chaplain resources when appropriate. * Provide comfort and reassurance to patients and family members * Assist Aspire nurse practitioners in managing their calendars when needed to manage urgent and emergent home visit needs * Various clinical and administrative tasks * Participation in weekly interdisciplinary team meetings * Other related duties as deemed necessary by Aspire Health * Active RN licensure required * Minimum of 3 years of nursing experience required, preferably in a centralized care management team environment * Prior healthcare experience serving chronically ill patients preferred * Fluent in Spanish preferred, but not required * Health plan or home health experience a plus * Telephonic case management or care coordination experience preferred * Strong customer-service orientation * Patient, compassionate and professional demeanor required * Exceptional organizational skills and ability to multi-task * Self-starter with an entrepreneurial spirit and demonstrated problem solving skills * Strong writing, Word, Excel, and database management skills * Extremely strong work ethic * Experience working in a fast-growing, rapidly changing environment Anthem, Inc. is ranked as one of America's Most Admired Companies among health insurers by Fortune magazine and is a 2018 Diversity Inc magazine Top 50 Company for Diversity.
