Overview1. To provide high quality professional nursing care as a member of the health care team to the patient population served by the office.

ResponsibilitiesCoordinates patient care activities and the flow of patient information by effective communication with other office staff, physicians, patients, families and a variety of health care agencies and appropriate documentation to ensure continuity of patient care. 1. Possesses ability to communicate effectively with providers, staff and patients to assist in identifying and solving problems. 3.

Assists with orientation of new staff when appropriate. 4. Conducts appropriate and timely documentation of patients' concerns, problems, and/or symptoms in the medical record. 5. Reviews medical records through office audit for complete and thorough documentation by provider reflecting patient's encounter. 6.

Assists or performs prescribed treatments/procedures ordered by the provider to help with patient's plan of care Ensures maintenance of a therapeutic environment that promotes effective and cost efficient patient care and provides for the safety of patients, staff and visitors. Coordinates and participates as an effective member of the health care team to ensure high quality care and patient satisfaction by providing a professional and congenial work environment for patients, office staff and physicians. 1.

Collaborates and integrates individual efforts with those of the other health care team members. 2.

Promotes a professional and congenial work environment. 3. Provides supportive nursing care to the patient through a friendly, caring manner. 4. Contributes and/or oversees process for ordering medical supplies and medications. 5.

Conducts effective, efficient, and timely telephone triage calls. 6. Maintains a clean and safe therapeutic work environment by applying policies and principles related to care of environment, safety, and infection control. 7.

Participates in educational opportunities and meeting to enhance job skills and increase knowledge 8.

Assumes leadership role by acting as a clinical resource person within the office setting. 9. Coordinates, organizes, and assesses the needs of patients and staff. 10. Provides support to the other members of the health care team Implements departmental nursing procedures and demonstrates nursing knowledge and skills by providing high quality, comprehensive patient care in an ambulatory setting, appropriate to the age and needs of the patient served by the office 1.

Assists and/or performs prescribed procedures in compliance with medical provider orders. 7.

Possesses knowledge and skills necessary to obtain adequate lab specimens for testing. 8. Performs in-office patient lab testing and quality control in compliance with medical provider's orders and office lab procedures/policies. 9. Conducts, performs and labels appropriate laboratory tests Provides patient education through application of teaching/learning principles appropriate to the age and needs of the patient population served by the office.

Associate's Degree in the field(s) of nursing required. 2. Bachelor's Degree in the field(s) of nursing preferred. Credentials: 1.

CPR 2. Registered Nurse in the state of IL Experience: 1. 1-3 years of experience in/with a medical ambulatory or office setting preferred.

VIEW JOBS 1/12/2019 12:00:00 AM 2019-04-12T00:00 Overview Transfer Coordinator * Registered Nurse and or Paramedic coordinates Patient Transfer Program which is a comprehensive transfer program for inbound and outbound patients. This role is responsible for ensuring that a seamless, efficient process is in place to accept and or transfer patients. Additional duties to market the services and build strong relationships with our referring hospitals, physicians and EMS providers in the primary and secondary market. Responsibilities Develop, implement, communicate and oversee various lists, programs, operations and procedures and visit offsite facilities. 1. Assist with completing the admission database as needed. 2. Demonstrate ability to plan, organize and implement CV Stat program goals and objectives in collaboration with the CV Stat Clinical Director, Sr. VP and VP of Cardiac Care Services. 3. Update physician and nurse mailing lists related to CV Stat program. 4. Maintain communication with referring areas on a regular basis. 5. Review daily schedule and hospital rounds list for accuracy and tracking. 6. Responsible for the development and implementation of educational programs for rural areas. 7. Visit rural sites as needed to maintain existing referral relationships and establish new referral sources. 8. Provide business office with any patient information requested for claims. 9. Oversee daily CV Stat operations. Maintain compliance with the departments' operating budget and provide ongoing management of program. 10. Serve as liaison between rural area physicians and Methodist Medical Center, fostering a commitment to return all patients to referring physicians for follow-up patient care. 11. Set up systems to track data and produce reports to administration as requested. Facilitate, manage, develop and maintain a variety of functions pertaining to the department. 1. Identify and monitor physician referral patterns to Methodist. 2. Facilitate physician communication according to established procedures. 3. Manage the referral physician relationship by physician office visits. 4. Develop and implement strategic plan for increasing physician referrals. 5. Develop a Methodist Cardiac Referral Network. 6. Involvement and attendance in Medical Functions which relate to developing relationships with referral sources 7. Assist in new business development of Physician Referral Patterns in the extended market. 8. Develop, maintain, and distribute materials for physician and office staff regarding CV Stat. 9. Develop and maintain the Hospital Physician Reference Guide. 10. Develop and maintain materials for the Physician & Office Orientation as needed to support the referral process. 11. Maintain knowledge of all Hospital Programs which related to referral sources. 12. Facilitate Physician Education: CME Courses and Symposiums. Promote the Grand Round programs and supply information of sessions. 13. Maintain knowledge of the CV Stat Transfer Process. 14. Maintain knowledge of the Inpatient Admission Process. 15. Maintain knowledge of Cobra/EMTALA Guidelines. Manage all CV Stat transfers. 1. Respond to all referrals to the CV STAT program in a timely manner. 2. Facilitate physician to physician to communication. 3. Communicate with sending facility to assure follow-up with Physician accomplished. 4. Assist sending facility with use of air or ground transport of patient. 5. Telephone receiving facility Admitting Department to admit patient. 6. Contact the receiving Nurse to outline the patient's condition, medical treatment plan and physical orders to prepare him/her for the patient's arrival. 7. Call physician's secretary to schedule any needed procedures. 8. Notify physician's scheduling department of patient's admission and scheduled procedures. 9. Notify CV STAT physician of E.T.A., patient condition changes and any issues concerning patient's care. 10. Maintain tracking record of all patient transfer information. 11. Produce monthly CV STAT program reports and forward to Administration. 12. Assist with patient and family education regarding procedures and condition as needed. 13. Collaborate with case manager regarding any special needs identified. 14. Communicate with sending facility representative and referring physician regarding patient status. 15. Assists with completing the admission database as needed. 16. Demonstrate ability to plan, organize, and implement CV STAT program goals and objectives in collaboration with the CV STAT Clinical Director, Sr. VP of Cardiac Service and the VP of Cardiac Care Service. 17. Update physician and nurse mailing lists related to CV STAT program. 18. Maintain communication with referring areas on a regular weekly basis. 19. Review daily schedule and hospital rounds list for accuracy and tracking. 20. Assists with development and implementation of educational programs of nurses. 21. Visit rural sites as needed to maintain existing referral patterns and establish new sites. 22. Provide business office with any patient information requested for claims. 23. Collaborate closely with Corporate Development and Marketing departments. Responsible for competencies, and developing staff along with maintaining team spirit and effective communication on the unit. 1. Uses resources in an efficient and cost effective manner. 2.
