We are looking for a Wound Ostomy Continence Nurse (WOCN) available to work 4 - 5 days a week (.8 to 1.0FTE). The FTE can be determined based on your need and can be either a .8 or 1.0 FTE. Work schedule is weekdays, 8:00AM - 5:00PM and no weekends! This position involves travel. The Princeton branch serves patients from Milaca to Brooklyn Center.

Responsibilities/Job Description

The Home Care Wound Ostomy Continence Nurse (RN) plans and delivers wound, ostomy, or continence services to patients using the nursing process of assessment, planning, intervention, evaluation and teaching. Provides consultation, education, and direct care for patients with needs in the areas of wounds, ostomies, and incontinence problems. WOC Nursing collaborates with physicians, nurses, and other health care disciplines to determine best treatment protocols, and facilitate quality outcomes for ostomy and wound care patients. WOC Nursing incorporates current research and best practice guidelines for the care of ostomy and wound patients.

Qualifications

Required:

  • Bachelor's degree in Nursing AND graduation from an accredited WOC Nursing program.

  • One year recent RN experience in acute care, home care, hospice or long term care.

  • Completion of WOC Nursing Internship as part of WOC Nursing Program.

  • Ability to function in job role independently.

  • Licensed as a RN and eligible to practice in Minnesota.

  • Certified Wound, Ostomy, Continence Nurse (CWOCN)

  • BLS for Health Care Providers certification.

Additional Requirements:

  • Valid Minnesota driver's license (or license that matches your current permanent residence).

  • Current and adequate automobile insurance policy.

  • Reliable transportation.

  • Computer skills (able to document effectively in EMR).

  • Interest and ability to lecture and provide staff education related to WOC nursing.

This is a casual role (0.01 FTE), meaning there are no scheduled/guaranteed hours per pay period. This a regional position and must be willing to work at both Wyoming, MN and Princeton, MN Responsibilities/Job Description This Patient Care Management Coordinator provides comprehensive care coordination of patients as assigned. The care coordinator assesses the patient's plan of care and develops, implements, monitors and documents the utilization of resources and progress of the patient through their care, facilitating options and services to meet the patient's health care needs. The intensity of care coordination provided is situational and appropriate based on patient need and payer requirements. This position is accountable for the quality of clinical services delivered by both them and others and identifies/resolves barriers which may hinder effective patient care. This position has responsibility to determine how to best accomplish functions within established procedures, consulting with leader on any unusual situations. Internal customers include all levels of nursing management and staff, medical staff, and all other members of the interdisciplinary health care team. External customers include physicians and their office staff, payers, community agencies, provider networks, and regulatory agencies.
