Job Summary

Provide quality, clinical services to patients in various settings in the community. Patient care is delivered within organizational philosophy, policy and standards of community health practice. The admissions process and case management responsibilities are performed in accordance with PHH policies and standards of practice, utilizing thorough and timely electronic documentation and interdisciplinary communications. Skilled nursing care is provided in the form of assessment, teaching, treatment, documentation and care coordination demonstrating high quality customer service, and financial awareness.

Qualifications and Experience

  • Graduate of an approved School of Nursing

  • Current Massachusetts license as Registered Nurse

  • Minimum of one year previous nursing experience in acute care or sub-acute care setting required.

  • Prior home care experience preferred.

  • Travel required within PHH's geographic area

  • Ability to work with various computer software required and knowledge of hand-held patient care computer devices preferred

  • Strong interpersonal and customer service and ability to work with a diverse population

  • Ability to communicate effectively in writing, verbally and electronically

  • Ability to work as a member of an interdisciplinary team of health care providers

  • Ability to organize and prioritize work, and adapt to changing situations

  • Ability to work independently, be self-directed and adapt to unpredictable circumstances.

Must be willing to work weekends/holidays/on call in accordance with department need/rotation

PRINCIPAL DUTIES AND RESPONSIBILITIES: Clinical responsibilities: Provides direct care to a select patient population in collaboration with the physician. Performs and records the patient history and physical exam and discusses findings with the physician. Utilizes advanced nursing knowledge to identify, prevent or solve complex and/or recurring patient care problems. Follows and manages patients (in collaboration with MD) enhancing continuity of care. Performs (oversees/assures completion of) specialized procedures particular to the select population mutually agreed upon with collaborating physician. Collaborates with unit staff and physicians to facilitate a comprehensive plan of care and coordinate resources. Writes orders which are in accord with current guidelines. Maintains and practices in accord with practitioner guidelines per state and hospital regulations. Monitors patient outcomes in accordance with defined standards of patient care. Identifies system-wide factors contributing to undesirable outcomes, such as complications, increased length of stay and overutilization of services. Documents patient visit within 48 hours of visit. Departmental responsibilities: Participates in the development of formal and informal educational programs for the unit-based nursing staff. Develops educational programs within are of expertise. Collaborates with nurse managers, clinical nurses and medical staff to plan, implement and evaluate pertinent change in practice and health care delivery. Facilitates patient movement through the system and efficient use of resources through interdepartmental collaboration. Coordinates coverage for time off unit, time off, and off-hours. Professional responsibilities: Responsible for own professional growth and maintenance of required credentials. Maintains the standard and educational requirements specific to the position and licensure. Identifies own learning needs and goals. Takes responsibilities for leadership and participation on committees and task forces of the DON and the hospital. Maintains and updates clinical knowledge and skills based on current health care practice/research. Applies evidence-based research. Supports research efforts to advance knowledge and to promote research-based practice. Evaluates own practice and program effectiveness. Identifies pertinent nursing and health delivery problems for investigation. Evaluates current research findings for clinical specialty and determines applicability to unit practice. Assists nursing and medical colleagues in research efforts. Independently seeks physician and supervisor responsibilities consultation. Maintains current CPR certification. Acts as role model for nursing service personnel. Seeks and is responsible for ongoing professional growth and development resulting in maintaining and securing active licensure. Maintains an awareness of pertinent developments in the profession and in clinical practice. Participates in at least one committee on an annual basis. Attends at least on MGH-offered learning workshop/training annually Assumes any other tasks as delegated. QUALIFICATIONS: * A registered nurse who has attained a Master's Degree in Nursing from an accredited program, and is certified by the ANCC or AANP to practice in the expanded role of nurse practitioner. * A minimum of two years nursing experience with specific populations. * Current, active professional licensure to practice in an expanded nursing role in the Commonwealth of Massachusetts. * A compassionate person with an understanding and appreciation of aging in today's society, family systems, and cultural influences. * Spanish language skills are helpful, but not essential. * Must have a telephone. * Acts only within the scope of his or her state license. * Must be able to demonstrate the knowledge and skills necessary to provide care to primarily population. * Must be able to demonstrate knowledge of the principles of growth and development over the life span. * Must be able to assess data reflective of a patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his/her age-specific needs. * Bilingual skills (Spanish & English) preferred, not required. * 5-10 year's experience preferred, required * Minimum of 2 Years of Adult NP experience required SKILLS/ ABILITIES/ COMPETENCIES REQUIRED: * Demonstrate competence in specialized area related to: * Telephone triage * Telephone care * Health education * Disease screening activities * LN use and application WORKING CONDITIONS: * Long periods of sitting.
