Job Summary:

The RN will work in the VPC department to support and provide clinical oversight, in conjunction with the Primary Care Medical Directors, and Director of Population Health, to the VPC care team. The RN will serve as an available resource for patients and clinicians, and act as a care guide to support patients through the continuum of care. The RN VPC Care Coordinator is responsible to carry out assigned nursing tasks and orders as delegated by the clinicians to support patient care.

Duties/Responsibilities:

  • Assists all patients through the healthcare system by acting as a patient advocate and navigator

  • Participates in patient-centered team meetings and quality improvement initiatives

  • Facilitates health and disease patient education

  • Supports patient self-management of disease and behavior modification interventions

  • Coordinates (in conjunction with other care team members) continuity of patient care with external healthcare organizations and facilities, including the process hospital admission and discharge and referrals from the primary care provider to a specialty care provider

  • Coordinates continuity of patient care with patients and families following hospital admission, discharge, and ER visits

  • Manages high risk patient care, including management of patients with multiple co-morbidities or high risk for readmission to a hospital setting

  • Conducts comprehensive, preventive screenings for patients and/or assists all support staff in daily patient interactions as needed

  • Promotes clear communication amongst a care team and treating clinicians by ensuring

  • awareness regarding patient care plans

  • Facilitates patient medication management based upon standing orders and protocols

  • Participates on a team for data collection, health outcomes reporting, clinical audits, and

  • programmatic evaluation related to primary care

  • Evaluates clinical care, utilization of resources, and development of new clinical tools, forms, and procedures

  • Follows standard procedures and protocols related triage and escalation of care

  • Follows protocols and acts on standing orders per clinician guidance to assist with refills,ordering preventative/screening interventions, etc. within RN scope of practice

  • Collect and analyze biometric data to support various population health and patient care initiatives

  • Provides support to medical assistants regarding clinical guidelines and protocols

  • Special projects, initiatives, and other job duties as assigned

Required Skills/Abilities:

  • Excellent verbal and written communication skills

  • Excellent interpersonal and customer service skills

  • Excellent organizational skills and attention to detail.

  • Excellent time management skills with a proven ability to meet deadlines

  • Proficient with Google Suite or related software

  • Sensitivity to Patient Concerns

  • Excellent knowledge of and ability to work with computer systems

  • Must be able to work independently remotely

Education and Experience:

  • 2-5 years experience in a clinical setting, ideally with chronic disease management care

  • Bachelor of Science in Nursing (Current RN license in state of residency. Additional multi-state

  • licensure may be required in other states which Doctor on Demand will facilitate.)

  • Experience working in an electronic health record system or have previous database

  • experience

Physical Requirements:

  • Ability to remain seated in a stationary position for prolonged periods

  • No heavy lifting is expected, though occasional exertion of about 20 lbs. of force (e.g., picking up and computer / laptops) may be required

  • Good manual dexterity required to use common office equipment (e.g., computers, laptop, and mobile devices)

We are looking for an experienced business executive to lead the growth and delivery of our expanding Virtual Primary and Specialty Care business. You will be a part of our extended executive leadership team. You will set strategic direction, manage performance against your product's P&L and lead the multidisciplinary efforts to deliver against growth goals, deliver an exceptional member experience and drive to strong clinical and financial outcomes while hitting margin targets. You work across the company and with our customers and prospects to ensure that your products succeed. You have the internal data and the market insights necessary to know how your product line is performing and you work to communicate that clearly to the rest of the company. You have strong points of view on what it takes to win and you hold the rest of the organization accountable to results. Duties and Responsibilities: * Own your 3 year business plan that defines the market opportunity, the goals that the product line can hit, the strategy to achieve the same and the resources that will be needed to be successful * Build and manage the business unit P&L, including partnering with the forecasting team to set the annual forecast for demand and associated clinical and operational capacity requirements * Own your product strategy and GTM strategy to propel market leadership: meet and exceed growth targets, meet and exceed client expectations, drive clinical and financial population health results, and continue to meet high standards for patient experience (4.9+ star ratings and MSAT >90) at the right margin targets * Scan industry trends and market dynamics to anticipate future direction of your product line in ways that allow us to remain on the forefront of the field * Know the competitive landscape and partner with GTM on competitive playbooks (i.e. takeout strategies), as well as driving win/loss strategies * Define product packaging and pricing that are market competitive and help us reach our product goals * Drive growth of the product line by owning the value proposition, messaging, proof points and go-to-market strategy * On an as-needed basis, GM will work closely with functional partners on key initiatives to drive the product goals, such as leading overall client delivery, partnering with marketing to drive utilization and working with product and clinical to drive MSAT and unit economics.
