    VIEW JOBS 6/19/2018 12:00:00 AM 2018-09-17T00:00 Overview Under the direction of the Site Director or designee, the PACE Interdisciplinary Team (IDT) Manager facilitates the care management activities for the PACE member's care/service plans. The IDT Manager ensures that the interdisciplinary team maintains appropriate and high quality care delivery and care management. The IDT Manager is the liaison and broker between the member, their family and the PACE IDT. The IDT Manager ensures the timely facilitation by the IDT the implementation of the core function of effective care management: ensuring that the IDT provides the assessment, planning, care coordination, implementation, advocacy, and evaluation/oversight necessary on all aspect of health care delivery to CLHC PACE member. The IDT Manager coordinates the IDT's care and services delivered to the PACE members: promotes effective utilization of resources, and assumes a leadership role within the interdisciplinary team to achieve optimal clinical and resource outcomes. Responsibilities I. Utilization Management Performs pre-authorization, discharge planning and review of inpatients with the IDT as stipulated by the guidelines and procedures outlined by the PACE Center Operations Department.Collects appropriate clinical information to make a determination of medical necessity for reviewed service based on Milliman Optimum Recovery Guideline and tracks variances.Begins the discharge planning coordination process with the IDT upon admission or precertification.Coordinates care with the member, family and IDT care team.Identifies specialized high risk cases with the IDT: Institutionalized, ESRD, transplant cases. Notifies other departments, as appropriate.Contributes to department goals and objectives in containing health care costs through the principles of utilization management II. Case Management Ensures that all PACE required assessments/reassessments are completed by the IDT members prior to care planning meetings.Facilitates IDT meetings and care planning meeting to ensure IDT consensus in the development of member-centered care plan.Responsible for the supervision of assigned clinicians, support, and administrative staff.Responsible for managing members in a telephonic disease/case management as needed, conducts face to face member visit/meetings with the IDT member(s) at home or other care delivery settings when needed. Communicates member's request or any clinical findings to the IDT for follow-up assessment/reassessment and care planning.Coordinates in conjunction with the primary care physician, IDT members, and other providers the specific needs for the memberEffectively communicates with patients, families, IDT, medical staff and caregivers to ensure patient's health needs are metAssists in the development of new disease management and complex case management programs and provides coaching of such management programs to appropriate IDT membersFacilitates the IDT's interventions to positively affect healthcare outcomes with emphasis on member's self-management of their disease stateManages program and information to measure and provide direction to the IDT members for continual improvement in access and quality of services to physicians and other health care professionals associated with the member's health planCreates and maintains logs of caseloads for reporting and to monitor effectiveness of interventions by the IDTMaintains HIPAA standards and confidentiality of protected health informationPromotes within the IDT members alternative care programs and researches available options including costs and appropriateness of patient placement.Maintains an active role in facilitating that the IDT assures the continuity of care for all inpatients through early discharge planning and that IDT members are working with hospital discharge planners, hospital social workers and the patient's physician in the early identification of potential home care candidates.Recommends, coordinates and educates providers on alternative care options.Works collaboratively with ancillary providers to execute agreed upon treatment plan with the IDT.Facilitates care delivery with the CHN & Nurse Manager in the provision of nursing care delivery such as start of care, care coordination, discharge planning and care management function to PACE members across the continuum of care settings. III. Quality ManagementContributes to department goals and objectives in maintaining a high quality medical delivery system through the principles of quality improvement.Collaborates with QI Coordinator to ensure compliance by the IDT to Federal and/or State regulations governing PACE. Collaborates with quality management to facilitate identification by the IDT of participants and coordination of services for participants in QI Projects.Identifies and presents potential quality issues to appropriate Health Plan committees for review and recommendationsIV. Data Management Ensures that all institutionalized patient stays, including retrospective cases and denied cases are completely and accurately recorded and documented by the IDT in the system in a timely manner.Oversees the data entry of IDT members to ensure that accurate and timely health care data and adequately documents patient related issues, (precert, concurrent review, alternative care, discharge planning, referrals), provider related issues and appropriate use of codes (ICD-9/10-CM and CPT Codes).Maintains appropriate logs identified by the Medical Management Department and monitors weekly for quality assurance of data entry accuracy. Follows through with concerned IDT member(s).Prepares and presents Care Management and Utilization Statistics (i.e., days/1000, referrals, HHC, etc.) to appropriate Health Plan committees. Maintains up-to-date member record including care plans, authorizations provided by the IDT, reports of services/interventions rendered by the IDT and/or other providers that includes progress notes and discharge summaries. Follow-through the resolution with department managers when performance issue is identified for a specific IDT member with regards to IDT responsibilities related to care provision, care coordination, documentation issues, and/or care management V. Special Activities Works with Provider Relations Representatives by communicating office/provider concerns, needs and expectations. * Serves as a liaison for PCP/PCP staff and the Health Plan.· Helps organize, manage and lead various committees and meetings.· Provides On-call nursing coverage as scheduled.· Performs other duties as requested by the Director and/or Vice President of Center Operations.Knowledge, Skills, and Abilities (Core Competencies):Communication: Maintains professional and positive interaction with IDT, physicians, offices, members and health plan staff. Monitor the IDT's implementation of necessary goals, interventions, and outcomes agreed upon during interdisciplinary team care planning/case conferences/meeting.Technical: Proficient computer skills. Knowledge of MS office including Word and Excel. Familiarity with use of nationally recognized medical criteria for determining medical necessity. CPT and ICD-9/10 coding. Qualifications Education: Baccalaureate degree in Nursing or a health related fieldrequired. Experience: At least two years direct patient care clinical experience required: in a Certified Home Health Agency, Long-Term Home Health Care Program, OR a Care Manager within an interdisciplinary team in an in-patient setting or a managed care organization. Minimum one year Manager/Supervisor level experience in an In-patient, Home Care, or Managed Care Company required. Previous Care Management Experience preferred. License Registration: Current active unrestricted license and registration in New York State required as an RN. Certification in Case Management from an accreditation organization or a certification in a health care related clinical field preferred. Other: Excellent written and verbal communication.
