JOB SUMMARY ##### The RN Telehealth Lead is knowledgeable regarding principles of tele-triage (an interpretation of client data to determine an appropriate course of action for early intervention) including symptom assessment, client education, communication with other health team members, and clinical decision making. Utilizes telehealth/telephone monitoring data to establish patient specific parameters, disease management protocols, and effective early intervention to prevent hospitalization. Identifies trends and subtle changes in patient condition from data and provides early intervention or change in treatment plan to promote disease management and prevent hospitalization.
, and providing potential organizational cost savings. # ESSENTIAL DUTIES AND RESPONSIBILITIES # Demonstrates broad knowledge of the rules and regulations governing home health. Utilizes evidence based nursing practices to set and maintain clinical standards of care and best practices in home health.
Serves as practice champion and clinical resource in initiatives to decrease hospitalization rates through collaboration and communication with clinicians. Supports implementation of best practices into clinical care. Collaborates with referral source, physician, intake, and case manager to identify telehealth/telephone monitoring appropriateness.
Implements telephone monitoring protocols for patients when telehealth is inappropriate for the particular patient or home setting. Monitors patients who are being treated as part of the Non-Invasive Ventilator program. Follows up with patient in response to dashboard alerts and reports off to overseeing Respiratory Therapists for those patients needing more specialized intervention.
Oversees the daily operations of Telehealth Services by coordinating time off of telehealth RN team (Rockford and Janesville) to ensure 7 day a week coverage and assisting Director manage Telehealth equipment inventory and maintenance.# Oversees the training of HHAs for competency in setup of equipment along with necessary cleaning procedures. Utilizes telehealth/telephone monitoring data of Home Health and Heart and Vascular Clinic patient to establish patient specific parameters, disease management strategies, and effective early intervention to prevent hospitalization. Utilizes this data to assess physiological response to changes in treatment.
Assesses and evaluates patient status incorporating telehealth/telephone monitoring data and trend reports to develop a plan of care specific to the individual patient. Identifies specific learning needs to reinforce, improve, or alter health behaviors and provides patient/caregiver education in symptom management, use of medications and treatments, diet and lifestyle modifications, and scheduling health care provider appointments. Participates in Nursing Practice Council and other practice initiatives to improve Clinical Practice and decrease hospitalizations.
Communicates regularly with case managers and physicians# utilizing trended data to facilitate making visits. Communicates regularly with HME Respiratory Therapists and Nurse Practitioners at the Heart and Vascular Clinic to facilitate early intervention of status changes as noted on dashboard. Organizes, prioritizes, and manages telehealth/telephone monitoring patient data in an effective manner serving as a resource for the case manager.
Maintains required records and documentation Must have transportation and ability to drive throughout agency service area. BSN or upon hire BSN completion agreement for enrollment in an accredited BSN program. Minimum of 1 year nursing experience in Home Health, Med Surg, cardiac services or related area required Experience working in Magnet environment preferred Experience with electronic health record software Proficient PC Skills, computer data entry skills and Windows software (MS Office) experience # CERTIFICATION/LICENSURE ######
Licensed as a RN in the State of Wisconsin required and licensed in the State of Illinois within 1 year of hire. If applicable, licensure to be applied for within 90 days of hire.
Approved CPR or BLS certification required within 90 days of hire Maintain a valid driver#s license and reliable transportation with proof of vehicle insurance when requested. Have and maintain an acceptable driving record per Mercy#s HR-01 Fleet Safety policy
ADDITIONAL REQUIREMENTS Passing the Driver#s License Check and/or Credit Check (for those positions requiring). Passing the WI Caregiver Background Check and/or IL Health Care Workers Background Check. Must be able to follow written/oral instructions.
