As a core member of the Care delivery team, the Clinical Nurse practices as the patients nurse and collaborates with the team to facilitate the coordination of patient care at the unit level. Through utilization of the nursing process, the clinical nurse diagnoses and treats patient and family responses to actual or potential health problems. The Clinical Nurse is accountable to the patient, family, peers, and Manager. Patient and family centered care (PFCC) at YNHH is demonstrated by working with patients and their families based on the 4 principles of PFCC: participation, dignity and respect, information sharing, and collaboration. This includes providing service excellence by creating a great "First Impression" by demonstrating exemplary customer service skills for all customer groups including patients, families/friends, physicians, staff and support department personnel. Responsibilities

    1. Nursing Process

  • 1.1 The Clinical Nurse I (CN I) utilizes the nursing process of assessment, planning, intervention and evaluation while caring for our patients maintaining confidentiality and dignity. The Clinical Nurse provides safe, effective evidenced based care tailored to the individual needs of the patient and family. As a core member of the care delivery team, the Clinical Nurse partners with the team to facilitate the coordination of patient care at the unit level utilizing the principles of patient/family centered care.

  • 3.1 Collaborates with members of the multidisciplinary team to support the work of the team, unit activities, and the accomplishment of positive patient outcomes.

  • 4.1 Delegates responsibilities to appropriate members of the care delivery team based upon an understanding of others' roles, scope of practice, knowledge, and capabilities to ensure optimal utilization of personnel.

  • 5.1 Successfully completes the competency based orientation within designated time frame.

  • 6.1 Communicates in a calm professional manner with patients, families, colleagues, members of the care team, and other hospital personnel, accepts and elicits feedback, and works toward resolving problems and clarifying issues and concerns.

Qualifications

EDUCATION

Graduation from an accredited School of Nursing., Licensed and a currently registered nurse in the State of Connecticut., All newly hired Registered Nurses must have current BCLS certification or must obtain within one month of hire.

EXPERIENCE

Clinical Nurse is the basic level for new graduates and all registered nurses., All RNs are expected to advance to their optimal level of functioning through promotion.

SPECIAL SKILLS

Excellent communications, interpersonal, coordinative, and organizational skills.

PHYSICAL DEMAND

Requires prolonged standing; walking; use of sight, hearing, and touch; moving/lifting patients of varying weights.

The Respiratory Therapist I is a health care professional who is a graduate of a Program in Respiratory Care approved by the committee on Accreditation for Respiratory Care (CoARC) and is licensed by the State of Connecticut as a Respiratory Care Practitioner. The Therapist I performs a variety of clinical activities in the assessment, treatment and education of the patient with respiratory problems. The Therapist I actively supports and serves as a resource to the other members of the patient care team. The Therapist I supports departmental and interdepartmental quality improvement efforts. Clinical Application of Respiratory Care: Functions efficiently, resourcefully and skillfully in providing patients optimum age-specific respiratory care. * 1.1 Assesses patient's pulmonary status employing basic skills, which include reviewing the medical chart, patient interview, physical examination, and interpretation of blood gases. Demonstrates proficiency and consistency with all assessment skills, including interpretation of x-ray and blood gases, RAW, CL graphic displays, etc. Demonstrates knowledge and interpretation of diagnostic procedures such as PPD, overnight oximetry, peak flow, arterial puncture, sputum induction. * 2. Documentation and Written communication: Contributes to effective communication within the department through documentation of services administered, changes in patients clinical status, and therapeutic objectives. * 2.1 Records all care provided via electronic medical record accurately. Identifies patient on written documents by using MRUN and patient labels as required by department and hospital policy . Notes consistently include ABGs, patient assessment, weaning assessment, and parameters. Maintains patient confidentiality with documentation practices. * 3. Consultation and Collaboration and Oral Communication: Communicates clearly and appropriately with co-workers. Demonstrates a cooperative team spirit, effectively contributing to excellence of patient care. * 3.1 Patient report information is appropriate and conveys current patient status. Gives report using SBAR format and attentively, courteously accepts report in order to facilitate transition from one shift to the next. Accurately, completely, and concisely gives a safe hand off for all relevant clinical information on patient's clinical course and status. * 4. Self-Development, Professionalism and Leadership - Assumes responsibility for maintaining clinical competency through continuing education, professional growth, and development. * 4.1 Assumes responsibility for professional self-development. Keeps self-informed about current trends within the profession and new procedures/equipment in the department. Requests inservices as appropriate to maintain competencies for equipment and procedures. Maintains membership in AARC, NBRC and Connecticut Thoracic Society or demonstrates other active participation in profession. Qualifications EDUCATION Graduation from an accredited program of Respiratory Care, current active license by the State of Connecticut as a Respiratory Care Practitioner and NBRC Registry Examination eligibility. NBRC Registration, Bachelor of Science Degree preferred. * Respiratory therapists employed by YNHH effective 9/12/2012 as a result of the acquisition of the Hospital of Saint Raphael may meet educational requirements with an active CT license as a Respiratory Care Practitioner and demonstration of current competencies. EXPERIENCE This is an entry level position for new grads plus those with some experience in critical care or neonatal environments.
