Responsibilities

  • Provide 1:1 care for client following client's plan of care at all times while maintaining client's dignity and independence

  • Monitor clients vital statistics and report abnormal findings to the case manager and physician assessments

  • Administer medications, feedings, oxygen, ostomy care as needed per client's plan of care and provided assessments of results

  • Complete routines as established by other care professionals such as Physical Therapist, Occupational Therapist, Speech Therapists

  • Ensure infection control policies are maintained at all times, report possible communicable diseases or infections and ensure the overall implementation and correct practice of infection control procedures

  • Accurately document nursing actions of all care given and communicate with family, case manager, and physicians. Also document the progress and outcomes for established goals and inform physician, case manager, and families of changes in client's medical condition and needs

  • Order supplies, medications, and feedings as needed to ensure adequate amounts are on hand at all times

  • Ensure all patient needs are being met in accordance with the instructions of the physician

  • Monitor incidents involving individuals. Ensure that incident reports are completed timely, accurately, and that case managers are informed. Examine the individual following the incident

  • Ensure all medical equipment is maintained and report faulty equipment to Durable Medical Equipment for repair

  • Other duties as assigned

Qualifications

  • Proof of current license as an Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice

  • Current Cardiopulmonary Resuscitation certification

  • Be able to bend stoop, push, pull, sit, and walk for periods of time

  • Able to utilize proper body mechanics while lifting 50 pounds

  • Knowledge of individual service plans, including understanding of client rights and health needs

  • Must have completed post secondary coursework to facilitate licensure as an Licensed Practical Nurse or Licensed Vocational Nurse

For Inpatient Services (Acutes), the preceding 12 months must be in a hospital setting. For Outpatient (Chronic), 2,000 hours in Dialysis in the preceding 2 years. * Proficient oral and written skills * Current CPR certification * Annual Physical * Two professional references from current / former supervisors * Citizen of the U.S. or proof to work in the U.S. Job Knowledge / Responsibilities: * Ability to utilize the nursing process in the provision of nursing care including but not limited to administering medications and treatments in compliance with the physicians orders and applicable hospital policy, interpreting diagnostic tests, formulating a plan of care, collaborating with other healthcare providers, and provide education to patients and / or significant others (while in compliance with HIPPA). * The RN must be able to demonstrate the knowledge and skills necessary to provide care appropriate to patients of all age groups. Must demonstrate knowledge of the principals of growth and development through the life span, possess the ability to assess data reflective of the patient's status, and interpret the appropriate information needed to identify each patients needs in relation to age. * Must document / chart observations and other data related to the clinical condition of the patient.
