GENERAL PURPOSE OF JOB

This position is responsible for working closely with onsite providers as well as both QuadMed and Client Leadership to manage all daily health and wellness center operations. This would include supervising all health and wellness center staff, including midlevel providers and support staff, as well as monitoring the daily operational processes for all health and wellness center staff. Ensures effective delivery of clinical patient care and compliance with administrative and clinical policies and procedures. Directs all prevention and wellness initiatives and oversees patient education, as directed by the providers.

KEY RESPONSIBILITIES

  • Actively supports and promotes the triple aim; improving the patient experience of care, improving the health of populations and reducing the per capita cost of health care.

  • Openly supports and consistently communicates the mission of the health and wellness center and the organization. Serves as the collective voice for health and wellness center staff to communicate with administration. Serves as the staff and patient advocate to ensure optimal patient care.

  • Interfaces with patients and families to ensure that care is consistent with their expectations. Ensures active use of the nursing process for patients throughout the health and wellness center visit. Serves as a professional role model and mentor to staff by responding positively to change while supporting colleagues through the change process.

  • Serves as a change agent who facilitates communication and supports the mission of the organization.

  • Provides leadership to direct reports through effective hiring, orientating, training, mentoring and coaching. Provides ongoing recognition, feedback and development, holding staff accountable for success. Identifies and mentors future organizational leaders. Creates an environment where employee engagement feedback facilitates positive change.

  • Monitors, measures, assesses and improves health and wellness center performance using patient satisfaction data and performance data, such as wait time and survey response rates.

  • Provides leadership to direct reports in the execution of work plans and long and short-term goals. Implements department level infrastructure and changes to supporting processes and systems. Ensures solutions to health and wellness center challenges are implemented. Sets, implements and completes health and wellness center goals.

  • Assures coordination/participation of health and wellness center staff on department and organization wide councils/committees.

  • Manages, supervises, and coordinates delivery of patient care. Ensures compliance with clinical patient care standards and established policies and procedures.

  • Maintains clinical competencies for clinical areas and provides patient care as needed. Assesses/coaches and evaluates staff level of practice. Develops programs that foster continued clinical development, from novice to expert, which will have a positive impact on patient care and the staff's career satisfaction.

  • Oversees various personnel actions including, but not limited to, hiring, performance appraisals, disciplinary actions, terminations and other related activities. Completes incident reports as needed and provides assistance regarding investigations and the resolution of complaints.

  • Plans, schedules and organizes work for the health center staff, ensuring proper distribution and delegation of assignments and efficient utilization of personnel, space and facilities. Overall accountability for the requisition, care and maintenance of unit inventory, equipment and supplies.

  • Coordinates the activities of staff. Accountable for the effective and efficient operation of the department and staff, including ensuring that orders, treatments, plans of care and clinical pathways are being carried out in a manner supportive of patient care and recovery. Ensure documentation of patient care is complete and accurate in accordance with QuadMed and department policies and procedures.

  • Collaborates with the Manager of Clinical Operations to determine fiscal requirements and prepares budgetary recommendations. Monitors the level of resources utilized in service delivery and determines the appropriateness of resource consumption in the service. Develops action plans in conjunction with the Manager of Clinical Operations to ensure budget and resource efficiency and satisfaction with patient care delivery.

  • Maintains confidentiality of all patient and organization information. Follows federal and state regulations and organization policies regarding protection of patient information.

  • Certain clients and health centers may require travel.

JOB REQUIREMENTS

Appropriate education and/or experience may be substituted on an equivalent basis

Education:

  • High School Diploma/GED required

  • Graduate from an accredited nursing program with an Associate's Degree. Bachelor's (BSN) degree preferred.

Experience:

  • Three (3) years of previous nursing and other direct reports leadership experience in an outpatient or occupational clinical setting

Certificates, Licenses, Registrations:

  • Current RN Licensure in state of residence or compact license

  • Current BLS CPR

  • PALS a plus

Knowledge, Skills & Abilities:

  • Bilingual in Spanish preferred, but not required

  • Knowledge of all regulatory, procedural, and licensing requirements

  • Knowledge of Lean Six Sigma guidelines

  • Proficiency in Microsoft Office (Outlook, Excel, Word) required

  • Requires great attention to detail, proficient management, patient education and excellent problem-solving skills

  • Familiarity with practice management software and electronic medical records strongly preferred.
