This position is responsible for planning, coordinating, integrating, educating and ongoing evaluation of the delivery of care to the Cardiovascular and/or Stroke programs. The coordinator is responsible for assuring the delivery of integrated Cardiovascular and/or Stroke care. Facilitates a seamless transition throughout the continuum of care in collaboration with patients, providers, vendors, and all stakeholders involved in the care of the patient and is responsible for assuring the team adherence to the program standards. Drives care coordination by actively providing consultative screening and evaluation of patients to direct and enhance access to IU Health services and improve patient, family, and referring provider satisfaction. Coordinates between patients, families, referring providers, and the care team. Assures and evaluates delivery of evidence-based care according to national, statewide, and organizational clinical practice guidelines; facilitates appropriate access; leads performance improvement initiatives; assists in developing clinical pathways; and assures care management occurs in a cost-effective manner while enhancing patient safety and the overall customer experience. Coordinates data collection with the registry team for IU Health.

• BSN preferred.

• RN with 2-4 years of experience required.

• Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.

• Previous Cardiovascular and/or Stroke patient care experience preferred.

• Able to collect and discern data, provide patient and staff education, and has a working knowledge of software applications.

• Able to function independently and use critical thinking skills.
