Overview

The Registered Nurse RN in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations and agency policies. Also, The RN Registered Nurse job coordinates total plan of care with other health care professionals involved in care, and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within a Point of Care setting.

Additional Details

  • Provides clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.

  • Makes the initial and|or comprehensive nursing evaluation visit, determines primary focus of care, develops the plan of care within State specific guidelines, and submits accurate, complete, and timely documentation, per policy.

  • Regularly evaluates and re-evaluates (as warranted by changes in condition but at least every 60 days) the patient's nursing needs.

  • Performs patient comprehensive assessments at designated time points and develops the appropriate POC, in collaboration with physician orders.

  • Ensures patients meet home health eligibility and medical necessity guidelines as defined by payer source.

  • Initiates, develops, implements and makes necessary revisions to the plan of care in collaboration with the physician and other health care professional's involved in care.

  • Makes referrals to other disciplines, as indicated by patient's assessed need.

  • Responds to outcome coordinator|coder and Patient Care Manager requests for clarification to OASIS assessments on the same day that the request for more information is sent.

QualificationsLicense Requirements

  • Current RN licensure in state of practice.

  • Current CPR certification required.

  • Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation.

VIEW JOBS 11/16/2018 12:00:00 AM 2019-02-14T00:00 PERFORMANCE PLANNING Provides direct nursing care to the residents and supervises the day-to-day nursing activities performed by nursing staff in accordance with current federal, and State regulations. MDS Coordinator: Responsible for over-all coordination of MDS process including assessment, care-planning and facilitating interdisciplinary team toward resident care goals and appropriate documentation of resident's progress. Ensures compliance with regulatory standards as it relates to MDS. KEY RESPONSIBILITIES / ESSENTIAL FUNCTIONS * Coordinates total nursing care for assigned residents. Assesses resident conditions, including physical, behavioral, psychosocial, developmental and age specific plans according to identified problems in a cost effective manner. * Directs LPN's and CNA's on a daily basis, including scheduling, supervising, evaluating, counseling, and daily operations. * Teaches, supervises and counsels the residents/family and caregivers regarding long term care procedures, residents' rights, and other care needs as appropriate to the resident's condition. * Develops and prepares the individual's plan of care, utilizing RAI guidelines in accordance with state, and federal regulations. * Performs duties based upon established standards of practice for adults and geriatrics. MDS Coordinator specific: * Facilitates the federally mandated Minimum Data Set (MDS) including resident interviews and portion of the MDS assessment within the nursing scope of practice and coordinates the assessments required by other disciplines assuring that all disciplines are aware of timelines for initial and ongoing assessments. Sets the assessment reference dates (ARD) and notifies appropriate disciplines of the ARD. * Computerized entry of MDS assessment data from all disciplines, final approval, and electronic transmission of the data to CMS. Validates success of transmission. * Develops the resident care plan in collaboration with other disciplines (RD, SW, PT/OT/SP, Restorative, RN, MD…..), consistent with the MDS and assures that the care plan is followed as evidenced by clinical observation and documentation. * Facilitates interdisciplinary care plan meeting. Coaches and educates resident care staff regarding the care plan, resident goals, implementation and evaluation of the plan and appropriate documentation that reflects the care plan and progress toward the resident's established goals. * In collaboration with other disciplines, updates resident care plan consistent with resident's assessed needs and communicates changes to resident care staff. Involves resident and significant other/family, as appropriate, in the plan of care. * Performs resident care rounds communicating with resident, significant other/family, and interdisciplinary care team members to evaluate and improve care, resident satisfaction, and environment. * Formulates an appropriate discharge plan involving the interdisciplinary team, physician, resident and significant other/family, as appropriate. Assists with arranging home care services and home medical equipment as appropriate. * Readily identifies changes in resident's condition that may warrant a change in level of care and takes appropriate action. * Collaborates with MDS nurses in order to gain efficiencies in processes, participates in educational opportunities. DCH Standards: * Maintains performance, patient and employee satisfaction and financial standards as outlined in the performance evaluation. * Performs compliance requirements as outlined in the Employee Handbook * Must adhere to the DCH Behavioral Standards including creating positive relationships with patients/families, coworkers, colleagues and with self. * Requires use of electronic mail, time and attendance software, learning management software and intranet. * Must adhere to all DCH Health System policies and procedures. * All other duties as assigned. MINIMUM KNOWLEDGE, SKILLS, EXPERIENCE REQUIRED Must be a registered nurse in good standing and currently licensed by the state of Alabama. At least five years experience as a Registered Nurse in acute care and/or skilled care setting with excellent patient assessment skills. Recent MDS experience with a good working knowledge of related federal guidelines preferred. Knowledge of skilled care reimbursement categories and documentation requirement to support RUG codes. Restorative Nursing experience in a skilled nursing setting (Swing Bed, Skilled Nursing, or Rehab unit) preferred. Wound care experience preferred. Excellent communication skills and demonstrated ability to work collaboratively and respectfully with all disciplines. Must be positive, energetic, self-directed, efficient, and have excellent communication and people skills. CPR certification up-to-date. Must possess basic computer skills in order to manage departmental application including MDS software system. Demonstrates communication skills for all population served. Positive attitude toward the elderly.
