Our nurses receive client specific training to our clients' complex medical needs and have a team of support 24 hours per day.

The RN (Registered Nurse) or LPN (Licensed Practical Nurse) ensures that services are provided according to the Plan of Care, licensing guidelines, professional medical standards and agency policies and procedures. This position requires the use of clinical judgement while supporting client safety protocols, caregiver training, and care collaboration.

Professional Responsibilities of the Home Care Nurse RN or LPN

  • Maintains clinical documentation according to agency policy and procedures and state/federal regulations

  • RN Strong assessment skills

  • LPN - Proficient in observation and monitoring of client clinical status

  • Strong organizational skills

  • Ambition to grow and advance clinically in independent setting

  • Deliver compassionate care to clients and clients' family (training, on-site supervision)

  • Ensure effective implementation of client services through the timely completion of required documentation as well as the timely transfer of pertinent medical data to the Nursing Supervisor, the client's physician, therapists, nursing team, or other caregivers

  • Current RN or LPN license in the applying state

  • Proactive approach to home health care management demonstrating leadership skills at all time

  • Must be able to lift, turn, position and transfer up to 50 pounds of weight

  • Prior nursing experience desired not required, preferably in acute cares, home health care, or public health services to help support the clinical demands

A team approach to care delivery is utilized. JOB DUTIES: * Providing direct and indirect skilled nursing care as assigned by Clinician or Registered Nurse. * Responsible to the Nurse Manager, Home Care, Clinician or Registered nurse for immediate needs. * Report significant changes in a patient's condition to appropriate persons in a timely manner. * Effectively communicate with residents, family and staff. * Demonstrates competency in skills required for the agency, including but not limited to, medical setups, wound care, and aseptic technique. * Provide ADL care as assigned in accordance with physician's orders and Home Health Aide Care plan. * Helps maintain continuity of care by working cooperatively with other health professionals. * Attends agency team care conferences, understands regulatory compliance and implements state licensure laws, JCAHO standards and Medicare Conditions of Participations (COPs). * Participates in the agency's quality improvement program: survey preparation, staff meetings, AWARE, CPR certified, yearly competency skills check list, home visit for competency. QUALIFICATIONS: 1. Current LPN licensure in the state of Minnesota. 2. Current BLS certification or able to obtain in three (3) months. 3. Home care experience preferred. 4. Demonstrates clerical competence, at least one year of nursing experience preferred. 5.Attends agency-provided inservice programs to fulfill requirements of position and agency policies. 6.Demonstrated ability to follow and adhere to Grand Itasca Clinic and Hospital policies, vision, mission and values. 7.
