Weekends - Part-time or PRN

The Registered Nurse (RN) in Home Health provides and directs provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations and agency policies. Also, The RN Registered Nurse job coordinates total plan of care with other health care professionals involved in care, and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources. All done within a Point of Care setting.

Additional Details

  • Provides clinical services within the scope of practice, as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team.

  • Makes the initial and|or comprehensive nursing evaluation visit, determines primary focus of care, develops the plan of care within State specific guidelines, and submits accurate, complete, and timely documentation, per policy.

  • Regularly evaluates and re-evaluates (as warranted by changes in condition but at least every 60 days) the patient's nursing needs.

  • Performs patient comprehensive assessments at designated time points and develops the appropriate POC, in collaboration with physician orders.

  • Ensures patients meet home health eligibility and medical necessity guidelines as defined by payer source.

  • Initiates, develops, implements and makes necessary revisions to the plan of care in collaboration with the physician and other health care professional's involved in care.

  • Makes referrals to other disciplines, as indicated by patient's assessed need.

  • Responds to outcome coordinator|coder and Patient Care Manager requests for clarification to OASIS assessments on the same day that the request for more information is sent.

  • Plots patient encounters for the episode and determines needed RN encounters based on patient's needs and regulations.

  • Instructs and supervises the patient's family|caregiver in the care of the patient and maintenance of a healthy environment for the patient.

  • Actively participates in weekly case conferences.

  • Maintains a current and accurate patient medication profile.

  • After start of care (SOC) assessment, reports the status of the patient, assessed needs, and plan of care overview to the team leader on same day (or by next business day if after hours).

  • Observes, records and reports to the physician and/or team leader the patient's signs and symptoms, response to treatment and changes in the patient's condition, as appropriate. Ensures adequate Team Leader (TL) communication when physician follow-up is needed.

  • Communicates changes in visit assignments, dates of scheduled visits, and schedule changes to scheduler and Patient Care Manger to ensure patient needs are met.

  • Communicates timely and effectively with agency personnel and ordering physician as required to process orders and OASIS data sets, schedule home visits, and deliver services to patient as ordered by physician and in accordance with applicable laws and regulation.

  • Facilitates hand-off communication to RN and PCM who will cover patients in their absence, prior to scheduled days off.

  • Performs regular and supervisory visits according to the plan of care and submits complete visit notes within 24 hours of completion visit.

  • Directly and/or indirectly supervises care provided by the home health aides and licensed practical vocational nurses, provides instruction as appropriate, and assigns tasks according to State and federal regulations.

  • Participates in on-call rotation.

  • Adheres to and participates in the agency's Episode Management process.

  • Assists in the orientation of new agency personnel as assigned.

  • Completes LHC required learning courses, additional assignments per DON request, as well as any state specific requiredtraining per state regulation|practice act requirements.

  • Participates in the performance improvement plan and process to ensure positive patient outcomes.

  • All other duties as assigned.

Qualifications

License Requirements

  • Current RN licensure in state of practice.

  • Current CPR certification required.

  • Current Drivers License, vehicle insurance, and access to a dependable vehicle or public transportation.

Our applicant will serve as a member of the nursing administration team and will be responsible for complete management and oversight of one of our four (4) Veteran Care Centers in our long-term care facility. Duties and Responsibilities (these duties are a general summary and not all inclusive): * Oversees day-to-day operational needs of the Veteran Care Center * Ensures continual delivery of high quality and effective nursing care * Plans for optimal utilization of staff for the delivery of nursing care throughout the 24 hour/7 day a week period. * Holds staff accountable for own clinical practice, including use of nursing process and appropriate documentation to meet all regulatory body standards. * Communicates effectively and resolves conflicts when they arise to promote trusting relationships * Adopts and implements innovative nursing practices to improve Veteran care * Participates in recruiting, selecting, mentoring and evaluating nursing personnel and their performance. * Ensures that clinical operations remain in compliance with professional and regulatory standards * Maintains a healthy and collaborative team environment * Develops and implements nursing policies and nursing * Maintains own current knowledge of the Veterans on the care center. * Maintains own competence in geriatric nursing and acts as clinical resource and expert role model to staff. * Assures staff knowledge and integration of age-specific nursing of the Veteran population as a critical element in the assessment, planning, implementation and evaluation of care. * Completes data collection and monitoring for quality and performance improvement. * Assesses, plans and evaluates nursing care and Veteran/family progress towards goals, directing staff as needed * Utilizes appropriate resources and exercises fiscal stewardship * Attends appropriate clinical committees to support Veteran care. Veteran Environment Management * Develops a care center's philosophy/goals which reflect the uniqueness of the care center and are consistent with the philosophy of both the nursing department and the Soldiers' Home in Holyoke. * Manages the care center fiscally by collaborating with the ADON/DON maintaining accountability for approved personnel, supplies and expense budget. * Maintains a safe environment for Veterans, staff members and visitors. * Works with other ancillary support services to the care center, initiating appropriate communication and planning to ensure a constant level of services to meeting Veteran care needs. * Prepares written reports on care center issues, trends and progress towards goals. Personnel Management * Participates in interview process for new employees; assists new staff members in orientation process and completes a competency-based skills inventory, and assigns senior staff to precept orientees. * Promotes teamwork with all staff assigned to shift. * Monitors activities of the Veteran Care staff for adherence to standards and need for education. * Promotes communication to staff members by conducting regular staff meetings to share information and maintains active communication by use of logs, etc. to personnel on the Veteran care center. * Promotes development of all staff members by: * meeting with each staff member on regular basis to evaluate performance, identify areas for growth, set mutual goals and provide progressive work assignments. * develops leadership skills in personnel by conducting/precepting in orientation of new personnel, and guiding the staff in the development of leadership abilities. * Conducts progressive discipline with individual staff members as needed, documenting appropriately. * Develops a Veteran Care Center based schedule and collaborates with other nurse coordinators/supervisors to assure balanced staffing patterns. Monitors use of sick time of staff. * Evaluates each member utilizing EPRS system. * Functions as facility supervisor on weekends and holidays if needed a rotating basis with other Veteran Care Coordinators. * Relieves other Veteran Care Coordinators, on a shared basis, during vacation, extended illness, etc. * Identifies goals for professional development and attends continuing education opportunities to attain goals. * Member of other SHH facility committees as needed to support nursing staff. * Performs other related duties as required or as directed by DON/ADON. Preferred Qualifications: * Proven ability to lead others and to effect sustainable change. * Bachelor's degree or higher in nursing * Certification in Geriatric Nursing or related areas. * Ability to supervise and inspire others including the issuance of corrective action when needed. * Ability to promote change and manage conflict * Knowledge of current evidence based practice and regulatory standards of care. * Knowledge of emergency nursing procedures. * Knowledge of methods in evaluating nursing practice and health outcomes * Intravenous skills and knowledge. About the Agency: The Soldiers' Home in Holyoke is a state-funded, fully accredited health care facility that offers veterans quality health care, including full-time residential accommodations, an on-site dental clinic, a multi-service outpatient department and provides educational and social services for approximately 5000 veterans. The Soldiers' Home includes a total of 280 beds consisting of 250 long term care beds, 30 residential beds and is staffed by approximately 350 administrators, clinicians, nurses, certified nursing assistants, skilled trade, dietary, housekeeping, contractors, consultants and general administrative personnel.
