• RN whom would provide personal cares to improve and/or maintain the senior residents’ physical and emotional well-being for our beautiful and active Assisted Living/Memory Care in Lake Elmo, MN. The team includes a Director of Health Services Nurse and Resident assistants trained to pass medications, as well as activities, culinary, housekeeping staff. This nursing schedule includes; -60 hours per pay period; Days, 8am

• 5pm

• Every other weekend and holiday rotation

• Monitors personal cares provided to assure consistency with each individual care plan

• Coordinates medications and performs treatments per physician orders and facility policies

• Assesses resident goals and revises, modify care plans as needed

• Provides reports and documentation related to incidents, change of status and cares provided
