Overview

The Fountains at Hosanna is looking for a Registered Nurse

• RN or an Licensed Practical Nurse

• LPN whom would provide personal cares to improve and/or maintain the senior residents' physical and emotional well-being for our beautiful and active Assisted Living/Memory Care in Lakeville, MN. The team includes a Director of Health Services Nurse, 3 other Licensed Nurses, Resident assistants trained to pass medications, as well as activities, culinary, housekeeping staff.

This full time nursing schedule includes;

• 72 hours per pay period

• Days/Evenings (two evenings shifts 2pm-10:30pm every 4 weeks)

• Weekends - every 4th

• Holidays - every 4th

Responsibilities/Job Description

• Monitors personal cares provided to assure consistency with each individual care plan

• Coordinates medications and performs treatments per physician orders and facility policies

• Assesses resident goals and revises, modify care plans as needed

• Provides reports and documentation related to incidents, change of status and cares provided

• Advise Resident Assistants as needed

Qualifications

• Current MN RN license or LPN License by start date
• AHA BLS for Healthcare Providers or Red Cross CPR for Professional Rescuers

Preferred:

• Comfortable using Microsoft windows and electronic charting systems

• Work experience in a patient support role, LPN or RN

• Good written and verbal communication skills to engage with residents
