Job Summary

Sign On Bonus - $5000. The Registered Nurse (RN) is responsible for utilizing the nursing process (assessment, diagnosis, outcomes/planning, implementation and evaluation) to provide professional nursing services to patients/residents at home.

The RN process includes the documentation of human responses to actual or potential health problems in the home health environment. This practice includes dependent, interdependent and independent functions. Collaborates with patient/resident and family, other inter-professional colleagues, such as physicians, to plan, implement and evaluate care.

The RN demonstrates competency and practices within the full scope of nursing expertise/knowledge and utilizes appropriate age and population specific standards as designated in their assigned setting. The RN cares for patients/residents in all phases of preventative care, health maintenance, diagnosis, treatment, and follow-up for patients/residents in the home setting. Responsible to implement the nursing process, the coordination and continuum of care, patient/resident assessment, patient/resident education, triage, and various other nursing interventions.

The RN functions within the scopes and standards of nursing practice as outlined in the Nurse Practice Act and Administrative Rules in state of practice and licensure. The Professional Nursing Practice recognizes the Scope and Standards of Practice and the Code of Ethics for Nurses with Interpretive Statements as published by the American Nurses Association as the foundation of nursing care delivery and professional conduct.

Qualifications

Graduate from a nationally accredited nursing program preferred, including, but not limited to, Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), and National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA). Bachelor's degree in nursing preferred.

Minimum of one year of professional nursing experience preferred.

Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Obtains and subsequently maintains required department specific competencies and certifications.

Valid driver's license required. Basic Life Support (BLS) required. Certificate for Outcome and Assessment Information Set (OASIS) Specialist - Clinical (COS-C) preferred.

Behavioral Health Case Manager I for Texas Medicaid is responsible for performing telephonic case management within the scope of licensure for members with behavioral health and substance abuse or substance abuse disorder needs. Primary duties may include, but are not limited to: * Uses appropriate screening criteria, knowledge and clinical judgment to assess member needs. * Conducts assessments to identify individual needs and develops care plan to address objectives and goals as identified during assessment. * Monitors and evaluates effectiveness of care plan and modifies plan as needed. * Supports member access to appropriate quality and cost effective care. * Coordinates with internal and external resources to meet identified needs of the members and collaborates with providers. * Field work may be required in this role. Qualifications Requires: * MS or MA in social work, counseling, or a related behavioral health field or a degree in nursing; 3 years clinical experience in social work counseling with broad range of experience with complex psychiatric and substance abuse or substance abuse disorder treatment; or any combination of education and experience which would provide an equivalent background. * Current active unrestricted license as an RN, LCSW, LMHC, LPC, LMFT, or Clinical Psychologist to practice as a health professional within the scope of licensure in state of Texas. * Experience in case management and telephonic and/or in person coaching with members with a broad range of complex psychiatric/substance abuse and/or medical disorders. * Experience coordinating community resources. * Experience using electronic documentation and experience using Microsoft Word, Excel and Outlook. Preferred: * Experience working with IDD population and/or IDD waivers. * Knowledge of Texas Medicaid. * Bilingual (Spanish) preferred.
