GENERAL SUMMARY AND SCOPE
Under the supervision of the Psychiatrist, administers the medication program and related policies and procedures; monitors all patients for whom psychiatric medications are prescribed; and coordinates the Central Pharmacy Program for the indigent chronically mentally ill. When necessary, provides initial assessments on new clients prior to their first appointment with the Psychiatrist. Provides crisis assistance and other assigned services as needed for the operation of the department commensurate with caseload and the achievement of productivity standards. Under the supervision of Health Home Director and Site Director, complies with all documentation and licensure agency policies and procedures. Maintains standards of professional nursing utilizing evidence-based practices.
As a member of the health home team, this person is responsible for the overall care management of persons on his/her caseload. Will conduct comprehensive assessments and coordinate all health home services, including the development, implementation and monitoring of the care plan and the participant's progress towards goals. Will provide education to participants on health care status, chronic diseases self-management, and maintenance of a healthy life style. Will manage and coordinate medical/behavioral health and social service needs. A team player who is flexible and able to listen to input from participants and other team members.
ESSENTIAL TASKS, DUTIES AND RESPONSIBILITIES
Provides diagnostic assessment and follow-up appointments for patients being discharged from the Inpatient Psychiatric Unit prior to prescriber appointments as needed. Provides crisis assistance and other assigned services as needed for the operation of the department commensurate with caseload and the achievement of the productivity standards.
Strictly adheres to all established policies and procedures, and evidence-based practice standards relative to the provision of Nursing Services and the Central Pharmacy Program.
Administers Central Pharmacy drugs and prescriptions to patients at appropriate time intervals, working within the projected Central Pharmacy budget to the extent possible.
Notifies the respective Site Director in a timely manner of any concerns related to the Central Pharmacy budget (over or under utilization of available funds).
Observes patients for recurrence of symptoms or possible need for medication adjustment.
Monitors records for possible misuse of prescribed medication claims. Alerts prescriber of any concerns and site director as needed to promote patient safety.
Reviews charts and maintains records in compliance with the standards developed by appropriate licensure agencies and continuous quality improvement criteria for the medication program. Completes progress notes in a timely manner. Makes recommendations to the Psychiatrist for ordering appropriate tests, etc.
Reviews physical health history forms and arranges for follow-up nursing appointment and/or referrals for medical care when appropriate in accordance with agency policy.
Provides ongoing monitoring of participants health care status.
Encourages participants adherence to medical and behavioral health recommendations including medication management.
Conducts case reviews and provides input to other team members.
Works in tandem and coordinates closely with Qualified Health Home Specialists to assure participant is receiving all needed health home services.
Interacts with community resources on behalf of the participant.
Tracks lab tests and follow-up as necessary.
Responsible for NOMS data collection activities on applicable clients.
Provides medical assistant duties on a PRN basis including rooming patient, collecting data regarding purpose of visit/chief complaint, drawing blood and collecting urine from patient for point of care and other laboratory testing.
OTHER TASKS, DUTIES AND RESPONSIBILITIES
Attends all health home meetings as required.
Attends staff and individual supervisory sessions as scheduled.
Dress professionally and maintain a professional demeanor with staff, clients, and community.
Attends case management team meetings for coordination purposes.
Participates in department performance improvement activities as assigned and attends required training.
Demonstrates competency at utilizing the department EMR as appropriate to role.
Completes time sheets and other administrative forms.
Demonstrates Service Excellence and ACE.
Performs other professional duties, as assigned.
KNOWLEDGE, SKILLS AND ABILITIES
Registered Nurse. Behavioral health experience strongly preferred. Holds valid Ohio professional license as appropriate.
Ability to demonstrate either formal training or a strong knowledge base in chronic physical health issues and the physical health needs of the SPMI population.
Experience working with individuals who have a mental, physical or drug and/or alcohol disorder is preferred.
Current CPR and BLS certification required and re-certification bi-annually.
Must possess a valid State of Ohio motor vehicle operator's license and be insurable under Firelands' auto policy. Employee must provide proof of personal automobile liability insurance coverage upon request.
Computer skills sufficient to learn and utilize the electronic medical record are a mandatory requirement.
Sensitive to the cultural needs of the individual and/or family served.
Punctual attendance at assigned work location is required.
Ability to work in a safe and efficient manner and maintain an accident free work place, including ability to demonstrate a working knowledge of emergency codes.
Ability to comply with OSHA regulations and CDC standard and transmission-based precaution recommendations and to utilize proper personal protective equipment. Ability to comply with provisions of applicable S.D.S. forms.
Successful completion of a ninety (90) day probationary period.
