Licensure required relevant to state in which work is performed. Paramedic obtained prior to hire date or job transfer date. Licensure required relevant to state in which work is performed. Pharmacist obtained prior to hire date or job transfer date. Licensure required relevant to state in which work is performed. Radiologic Technologist specializing in Radiography credentialed from the American Registry of Radiologic Technologists (ARRT) obtained prior to hire date or job transfer date. Licensure required relevant to state in which work is performed. Registered Nurse obtained prior to hire date or job transfer date. Licensure required relevant to state in which work is performed Rehab Therapist specializing in Occupational Therapy (OTR) obtained prior to hire date or job transfer date.
