Responsible for completing Annual Wellness Visits with eligible patients. Ensures education provided to patient is factual and applicable to the individual.# Monitor and manage patients# Medication Adherence with various payers. Assists patients with identifying and overcoming barriers they may face that prevent adherence to medications. # Must have licenses#to practice as a Registered Nurse in North Carolina, BLS/CPR through the American Heart Association and#ACLS certification.
