The registered nurse will work in a collaborative, interprofessional team caring for patients with substance use disorders and perinatal behavioral health. The RN will work in the following settings: adult outpatient substance use disorder bridge program and outpatient perinatal behavioral health program.

The duties will include supporting the physician(s), advanced practice clinicians and licensed clinicians within these programs. The RN will be responsible for rooming patients including vital signs, performing appropriate screenings, medication reconciliation, ordering and collecting drug testing, and telephone support to patients. In addition, the RN will be responsible for some routine patient follow-up visits.

These visits will include assessments of patients, their progress towards meeting treatment goals, and appropriateness of current treatment plans with oversight from Medical Director of Addiction Medicine and APCs. The RN will also have an important role in working with licensed clinicians and other staff to help coordinate referral to community treatments resources.

ESSENTIAL FUNCTIONS

1.Performs clinical nursing responsibilities in adult medicine, addiction, and obstetrics.

a. Complete set of vitals on each patient

b. Collect toxicology testing.

c. Phlebotomy as needed.

d. Administer appropriate vaccinations and medications.

e. Assist in contraceptive management, including LARC (long acting reversible contraception) device placement.

f. Provide pregnancy education.

g. Completed forms on behalf of patients for various needs.

2.During program hours actively manage patient flow, including rooming and triaging.

3.Help identify and triage level of care and intervention needed in cases of substance intoxication or withdrawal Support prescribers in providing outpatient pharmacotherapy for addiction, utilizing pharmacotherapies appropriate to clinical needs of the patient, including opioid antagonist or agonist therapy such as naltrexone, buprenorphine and methadone.

4.Work with a multidisciplinary team of providers to devise an individualized plan for optimal medical and psychosocial treatment for individual patients and their families.

5.Preparatory work prior to program day includes:

a. Facilitate necessary testing.

b. Order labs/medications.

c. Prepare huddle notes for each patient prior to his/her appointment.

d. Order vaccinations.

6.Review lab results and alert providers as needed.

7.Call patients to review results.

8.Monitor supplies and manage ordering as necessary.

9.Act as tobacco education coordinator



  1. CASE MANAGEMENT RESPONSIBILITIES

a. Engage patients in treatment planning and make appropriate referrals/complete necessary communications

b. Work collaboratively with program staff to assist patient with securing additional addiction treatment (e.g. intensive outpatient, residential, detox placement)

c. Collaborate and consult with patient, families, professionals, and providers within and outside of the hospital to develop and implement inpatient, post hospital, and outpatient care plans.

d. Make referrals to all levels of care by providing clinical information and documentation to support patients' transfer to other facilities.

This may include drafting letter of engagement and preparing information to fax, copying portions of the medical record, and scheduling intake appointments for patients. It will also include arranging outpatient follow-up appointments for patients being discharged, which may include helping the patient to obtain a new PCP.

e. Complete follow up phone calls to patients to ensure patient engagement with community partners.

f. Develop strong, effective working relationships with outside agencies, facilities, payors, and other relevant community resources to facilitate disposition solutions for patients and families.

g. Compile and organize treatment resources that are easily accessible to care team members.

h. Provide education and information about available resources and SUD services and explore patients' goals of care and provide support to lower barriers to help them to achieve those goals.

i. Adhere to federal and state policies and ensure appropriate releases are obtained to exchange and disclosure necessary patient information.

j. Manage releases of information and general communications between the program and outside organizations including Department of Children and Families and other treatment providers.



  1. OPERATIONAL RESPONSIBILITIES

a. Serve as point person for referrals and intakes.

b. Provide occasional phone coverage for the program.

c. Work with the program directors and patient services coordinator to manage insurance issues and prior authorizations as well as any other pharmacy or prescription challenges.

d. Work with multidisciplinary team to improve program workflow and best practices.

e. Collaborate with inpatient team in treatment planning for program/OB patients.

JOB REQUIREMENTS

Minimum Education

  • Preferred

Graduate of an accredited School of Nursing, Bachelors degree preferred

Minimum Work Experience

Five (5) years of nursing experience; addiction experience is strongly preferred. Prior work at Opioid Treatment Programs (OTP), Acute Treatment Services (ATS)/detox, clinical stabilization services (CSS) is preferred.

Comfort working with people suffering from substance use disorders. This should include responding to acute overdoses, providing naloxone, and destigmatizing medications for addiction treatment.
