Job Summary:

This position is responsible for performing RN duties on an as-needed basis using established guidelines to ensure appropriate level of care. Performs duties and types of care management as assigned by management.

Responsibilities:

  • Completes all assigned tasks and functions as assigned by management in a timely manner.

  • Assesses patient's clinical need against established guidelines and/or standards to ensure that the level of care and length of stay of the patient are medically appropriate for inpatient stay.

  • Evaluates the necessity, appropriateness and efficiency of medical services and procedures provided.

  • Coordinates and assists in implementation of plan for members.

  • Monitors and coordinates services rendered outside of the network, as well as outside the local area.

  • Coordinates with patient, family, physician, hospital and other external customers with respect to the appropriateness of care from diagnosis to outcome.

  • Coordinates the delivery of high quality, cost-effective care supported by clinical practice guidelines established by the plan addressing the entire continuum of care.

  • Monitors patient's medical care activities, regardless of the site of service, and outcomes for appropriateness and effectiveness.

  • Advocates for the member/family among various sites to coordinate resource utilization and evaluation of services provided.

  • Encourages member participation and compliance in the case/disease management program efforts.

  • Documents accurately and comprehensively based on the standards of practice and current organization policies.

  • Interacts and communicates with multidisciplinary teams either telephonically and/or in person striving for continuity and efficiency as the member is managed along the continuum of care.

  • Understands fiscal accountability and its impact on the utilization of resources, proceeding to self-care outcomes.

  • Evaluates care by problem solving, analyzing variances and participating in the quality improvement program to enhance member outcomes.

Knowledge, Skills and Abilities:

  • Prefers proficiency in the use of personal computers and supporting software in a Windows based environment, including MS Office products (Word, Excel, PowerPoint) and Lotus Notes; prefers knowledge in the use of intranet and internet applications.

  • Prefers working knowledge of case/care management principles.

  • Prefers working knowledge of principles of utilization management.

  • Prefers basic knowledge of health care contracts and benefit eligibility requirements.

  • Prefers knowledge of hospital structures and payment systems.

  • Knowledge of Horizon systems, processes, and operations.

Education:

  • Requires an associate's or bachelor's degree (or higher) in nursing and/or a health related field OR accredited diploma nursing school.

Experience:

  • Requires a minimum of two (2) years clinical experience.

  • Prefer experience in managed care, care management and/or utilization management.

  • Horizon Clinical Operations experience as RN II or higher

Additional licensing, certifications, registrations:

  • Requires an active New Jersey Registered Nurse License.

Laurel Function:Clinical New Jersey MENTOR Description PsychiatricAdvanced Nurse Practitioner (APN) New Jersey MENTOR is currently seekinga passionate Psychiatric Advanced Nurse Practitioner (APN) to provide office-basedinitial assessments and monthly medicine reviews with youth in our programs. As the APN- Advanced Nurse Practitioner you will providetotal nursing care to patients under the direction of a physician. Thisrole will administer prescribed medications and treatments in accordance withapproved nursing techniques. You will also observe and evaluate patients,record condition and reaction to drugs, treatments, and significant incidents. Per DiemSchedule: 1 week day per week required from 12-6pm (flexible with yourschedule) Youth arescheduled every 30 minutes for medication review and 1 hour for initialassessments. Responsibilities: * Evaluate individual health care status of patients, including eliciting patient history, performance of physical examination, ordering of screening and diagnostic tests, and risk assessments. * Interpret and evaluate diagnostic tests to identify for a patient's clinical problems and health care needs in order to establish a diagnosis. Record physical findings, and formulates plan and prognosis, based on patient's condition. * Discuss case with physician and other health professionals to prepare comprehensive patient care plan. Submit health care plan and goals for periodic review and evaluation by physician. * Monitor the individual's response to therapy/medication and revise the plan, as indicated, with particular attention to education and counseling and encouragement of self-care. Care for the patient within the context of the patient's life and culture. * Ensure that current and complete medical records and reports are in accordance with regulatory and corporate standards, including medication administration records and physician's orders. * Educate clients and/or families about preventive care, medical issues, and use of prescribed medical treatments and/or medications. * Recommend drugs or other forms of treatment such as physical therapy, inhalation therapy, or related therapeutic procedures. * Administer prescribed medications according to established guidelines and approved nursing techniques. Provide documentation for all medications given. * Participate in quality and process improvement activities to improve individual care and/or related systems. * Perform other related duties and activities as required.
