RN PDN Supervisor Westminster-Co

Amazing Care Home Health is seeking a full-time PDN Supervisor for our Westminster, CO office. As we expand, we need a dedicated individual to join our team and uphold our commitment to compassion and quality service.

Job Summary:

The PDN Supervisor will play a crucial role in overseeing the care management for their assigned caseload and clinicians in the Private Duty Nursing Department, ensuring the delivery of high-quality care to our patients.

Responsibilities:

• Coordinate all aspects of patient care, including recertification, admission, treatment, and discharge planning

• Provide clinical and clerical management support

• Collaborate with case managers and clinical field personnel to ensure optimal patient care

• Work autonomously and demonstrate strong leadership skills

• Work a flexible schedule, with a minimum of two days in the office per week, preferably Mondays and Tuesday

• Exhibit motivation and eagerness to continually grow skills, with flexibility to adapt to changes

• Demonstrate strong communication skills and the ability to navigate obstacles

• Act as a team player and be willing to commute to patient homes when needed, assisting with visits as required

Qualifications:

• Current Supervisory/Case Management experience

• Experience in home health and a management role

• Ability to work independently and prioritize tasks effectively
