The Hospice RN or LPN makes visits to supplement the nursing care needs of our patients/families.

Responsibilities

Responding to phone calls promptly
Making visits as necessary
LPN - carrying out physician orders under the direction of a RN
Informing the physician of changes in the patient's condition and needs
Communicating with medical professionals, the hospice team and others
Maintaining accurate, up-to-date records
Elevating necessary matters to the attention of the patient care manager
Demonstrating strong communication and customer service skills

Requirements

RN-current Oklahoma nursing license
LPN-graduate of an accredited LPN program / current state license as a Licensed Practical Nurse
Hospice experience required
Valid driver's license

Responsibilities: The Monitor Technician is responsible for recognition and interpretation of cardiac rhythms in a facility with a patient population of ages ranging from 18 years to geriatric experiencing medically complex conditions, general medical-surgical conditions, with rehabilitation and/or wound care needs. * Assume responsibility for watching monitors continuously. * Monitors heart rhythm patterns of patients on telemetry or cardiac monitoring equipment and reports outcomes. * Responds to alarms and immediately notify Licensed nurses of significant change, condition or rhythm. * Runs EKG strips on shift as well as provide initial EKG interpretation * Monitors other alarms and call lights as requested. * Perform additional duties assigned. Minimum Qualifications: * Ability to project a professional image. * Working knowledge and ability to apply professional standards of practice in job situations. * Organizational and analytical skills. * Working knowledge of personal computer and software applications used in job functions (Word processing, graphics, databases, spreadsheets, etc.) * Persons who have been found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care related setting are ineligible for employment in this position. * Freedom from illegal use of drugs. * Freedom from use of and effects of use of drugs and alcohol in the workplace. Education and/or Experience: * High school diploma or equivalent required. * Successful completion of a Monitor Technician Course or Basic Arrhythmia Interpretation Competency is required. * Previous experience as a Certified Nursing Assistant, EMT or paramedic preferred. Perform delegated tasks after instruction by RN or PT. Bathe clients, assist with showers and partial baths as appropriate. Assist clients with proper exercises and assist PT in the rehabilitation for client when applicable. Help client maintain personal hygiene and assist with all aspects of activities of daily living. Answer client questions about self-care techniques and give instructions that will assist family, client, and family/support in providing care. Assist client in getting ready for doctor appointments. Lift, turn, and weigh clients as appropriate. Take and record vital signs as assigned, including blood pressure, temperature, respiration. Qualifications Certified Nursing Assistant (CNA) strongly preferred. Must be registered and listed in good standing on the Nurse Aide Registry in the state of employment. Minimum of 1 year of experience in a home health or hospice setting. Minimum of 1 year of full-time experience in direct client care in an institutional setting (hospital or nursing facility). Must have the ability to recognize the needs of people, to maintain good relationships with people, and have the capacity to make meaningful observations and communicate them to the supervisor.
