RN Navigator

The RN Care Navigator is responsible for managing patient populations with acute, chronic illness and/or multiple co-morbidities, identified as requiring additional transition of care support following visit to the Emergency Department setting. This individual works in coordination with multiple PCPs and/or community providers and serves as the central link to an identified population of patients.

Required Qualifications:

• Graduation in Nursing, from an accredited school.

• Washington Registered Nurse License upon hire.

• National Provider BLS - American Heart Association upon hire.

• 2 years

• RN role in the Emergency Department setting.

Preferred Qualifications:

• Bachelor's Degree

• Nursing.

• Certified Emergency Nurse (CEN) upon hire.

• Experience managing complex patients in an Emergency Department setting.
