Job Summary
The RN Navigator plays a pivotal role in guiding patients through their healthcare journey, ensuring they receive high-quality, efficient, and cost-effective services. This position involves the development and maintenance of department-specific programs, working closely with both internal and external departments to establish consistent work standards, identify community resources, and coordinate educational platforms for all patient types. This role is essential in ensuring the seamless delivery of healthcare services, enhancing patient satisfaction, and maintaining high standards of care. The Prevea Clinic Navigator is dedicated to supporting patient populations and their providers, promoting a collaborative approach to healthcare, working in partnership with the entire healthcare team to manage transitions of care between Prevea, HSHS, and other healthcare partners.
What you will do
Assist and support the department navigation program goals. Collaborate with providers and staff to develop standardized protocols and workflows to address needs and care gaps.
Assess barriers to care such as population issues, gaps in care, and lacking resources. Work in collaboration with internal and external departments to meet those social determinants of health.
Coordinate and facilitate care for the seamless delivery of care between internal and external departments. Tasks may include: -Review and complete referral intake
Initial assessment and triage of patient based on created clinical practice guidelines
Collaborate with multidisciplinary team to create and implement patient care plan
Follow patient through continuum of care to serve as a resource for both the patient and the multidisciplinary team
Assist with appointment scheduling as needed
Complete patient education and continued triage of patient symptoms to identify necessary changes to plan of care
Communicate with patients and providers in an appropriate timeframe.
Provide patient education using professional judgment within the scope of license.
Triage patients to the appropriate setting.
Participate in internal and external community advocacy and programming
Work within EMR to research and document care plans

Ability to form work/patient relationships with patients and peers
Effective interpersonal skills, strong written and oral communication as well as strong listening skills
Understanding of community resources for appropriate patient population
