This job plans, organizes, develops, and coordinates Minimum Data Set (MDS) assessment of patient in the Skilled Nursing Facility. Responsible for compliance with all state and federal regulations regarding the MDS (i.e., transmission, adherence to completion dates, etc.) and collaborates with both clinical and administrative staff, regarding the Minimum Data Set (MDS).

Knowledge Skills and Abilities (KSAs)
Must have computer skills and dexterity required for data entry and retrieval of required patient information.
Must be proficient with Windows-style applications, keyboard and various software programs specific to role.
Effective verbal and written communication skills and the ability to present information clearly and professionally to varying levels of individuals.
Skills and ability to operate MDS software package
Good organizational skills.
Ability to work behond normal hours, weekends, and in other apprpriate positions temporarily as needed.
Demonstrates effective use of designated assessment and data collection tools to maintain current patient records in accordance with state and federal guidelines.
Assumes responsibility for transmission of current assessment records in accordance with CMS guidelines.
Communicates effectively with Unit Director MDT (multidisciplinary team), unit staff, and hospital staff Administration.
Ensures high standard for the Minimum Data Set
