Full-time position with competitive wages and excellent benefit package. At least one year of MDS 3.0 exp and LTC exp required. Will be required to take call every 4-5 weeks.

DUTIES

JOB KNOWLEDGE AND ROLE RESPONSIBILITIES:
1.The MDS Coordinator is responsible for the timely and accurate completion of the Federal and State assessment tool. Position serves as an active member on the interdisciplinary and management team.
2.Demonstrates knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in planning delivery of care.
3.Maintains comprehensive and up-to-date knowledge of the MDS rules and regulations as described in the MDS User's Manual, including item coding, RAPS, care planning, electronic submission, scheduling, etc.
4.Committed to an interdisciplinary treatment approach to rehabilitation by exercising sound nursing judgment based on preparation, knowledge, skills, understanding and past experience in nursing care situations.
5.Accountable for compliance and consistent application of the Code of Conduct and Compliance Program, Resident Abuse Reporting and Prevention Policies, and State and Federal Code requirements pertaining to long-term care nursing service.

QUALIFICATIONS

SUMMARY OF QUALIFICATIONS:
1.A graduate of an accredited school of Registered Nursing and currently licensed as a Registered Nurse in the State is required.
2.Previous work experience in a long term care setting. Supervisory experience and organizational skills in a long term care setting preferred.
3.Is listed in good standing as confirmed on the State Department of Professional Credentialing.
4.Possesses excellent technical assessment and documentation skills, and leadership qualities.
5.Possesses good communication skills and is patient and self-disciplined.
6.Must be able to communicate in English, both verbally and in writing, and possess sufficient communication skills to perform the tasks required.
7.Knowledge of MDS rules and regulations including item coding, RAPS, care planning, electronic submission, scheduling, and working knowledge of relationship between PPS and MDS, etc.

VIEW JOBS 1/15/2019 12:00:00 AM 2019-04-15T00:00 General Description for Staff Development Coordinator Under the direction of the Director of Nursing Services and/or Executive Director, performs recruitment, staffing and training functions in accordance with Facility policies and procedures. Essential Duties 1. Contributes to Facility quality assessment and improvement initiatives. 2. Coordinates, conducts and tracks Facility education program, including mandatory in-services and nursing staff competencies required by state and federal regulations, Facility policies, and QA-identified needs. 3. Prepares nursing schedule which adheres to the Facility's staff-to-resident ratio. 4. Reviews and approves requests for time off based on staffing needs, as needed. 5. Administers Facility work rules, policies and procedures and collective bargaining agreements (if applicable) in the hiring and scheduling of staff. 6. Schedules and conducts interviews with both internal and external nursing department candidates. 7. Trains and orients nursing department new hires (classroom and floor orientation). 8. Performs pre-employment drug screening, reference checks, and background checks as needed. 9. Administers and tracks employee and resident vaccine requirements per regulations and Facility policy. 10. Maintains records of applicants not selected for positions per retention guidelines as needed. 11. Educates staff on required policies and procedures. 12. Maintains secure personnel records for employees as needed. 13. Reduces unscheduled vacancies by contacting available staff to cover shifts, as needed. 14. Supports adherence to Facility attendance, paid time off, leave of absence and FML policies. 15. Manages Tara University database as directed. 16. Advises the Supervisor of outstanding issues, potential problems, and labor relations matters. Standard Requirements 1. Supports the Mission, Values and Vision of the Facility. 2. Supports, cooperates with, and implements specific procedures and programs for: OSHA and safety, including standard precautions and safe work practices, established fire/safety/disaster plans, risk management, and security, report and/or correct unsafe working conditions, equipment repair and maintenance needs. 4. Adheres to: a. Confidentiality of all data, including resident, employee and operations data. b. HIPAA regulations c. Quality Assurance requirements and compliance with regulatory requirements. d. Compliance with current law and policy to provide a work environment free from sexual harassment and illegal and discriminatory behavior. e. Code of Conduct and Compliance Program when performing work functions 5. Supports and participates in common teamwork: a. Cooperates and works together with all co-workers; plans and completes job duties with minimal supervisory direction, including appropriate judgment. b. Uses tactful, appropriate communications in sensitive and emotional situations. c. Follows up as appropriate with supervisor, co-workers or residents regarding reported complaints, problems and concerns. d. Promotes positive public relations with residents, family members and guests. e. Completes requirements for in-service training, acceptable attendance, uniform and dress codes including personal hygiene, and other work duties as assigned. f. Participates as assigned in the orientation of new staff and demonstrates a positive, helpful, and enthusiastic attitude. 6. Immediately reports incidents of alleged resident abuse or neglect or alleged violations of residents' rights to Supervisor or Executive Director. 7. Knowledgeable of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive and socialization factors) in planning delivery of care. 8. Maintains current practice standards or changes by participating in educational opportunities provided by the Facility and outside resources. Other Duties Provides hands on nursing care to residents as needed as a registered professional nurse or licensed practical nurse, consistent with the state's Nurse Practice Act. Perform tasks which are supportive in nature to the essential functions of the job, but which may be altered or redesigned depending upon individual circumstances. Performs other work duties as assigned. Qualifications Education/Training: Valid State-appropriate Nursing license required A.D.A ((Americans with Disabilities Act)(as amended)) REQUIREMENTS: Must be able to bend, stoop, push, pull, and walk and stand for most of the working day. Requires manual dexterity sufficient to operate standard office machines such as fax, telephone, etc. Requires normal range of hearing and vision. Must be free from communicable diseases that pose a "direct threat" to the health or safety of others and which cannot be eliminated by a reasonable accommodation. Must be able to lift a minimum of 50 pounds and transfer and transport residents, as applicable, and be able to carry and transport supplies.
