POSITION SUMMARY:

Provision of professional nursing care to patients includes comprehensive assessment of patient and family needs, planning, coordination, teaching, counseling, and direct physical care. Evaluation of patient response to care and progress toward outcomes is an essential aspect of care provision.

Assists other personnel providing delegated aspects of care. Performs other related duties as assigned. Functions as a member of the interdisciplinary team.

QUALIFICATIONS:

Required knowledge and skills:
1.Provide age-appropriate care;
2.Develop interpersonal relationships with patients and families with an emphasis on caring;
3.Computer knowledge required;
4.Effective communication skills with ability to listen both verbally and non-verbally;
5.Works independently and takes initiative on behalf of the patient;
6.Manages time effectively;
7.Demonstrates accountability and responsibility for safe patient care.

Education and experience:
1.Associate degree (ADN) from an ACEN or CCNE accredited school of nursing or diploma in nursing required; Bachelor's degree (BSN) from ACEN or CCNE accredited school of nursing desired

Licensure, registration or certification:
1.Current Registered Nurse licensure in the state of Kansas.
2.Certification in CPR/Basic Life Support.
3.Recommended Certification in Advance Cardiac Life Support per unit discretion.
4.There may be additional certifications necessary dependent on the unit assigned.

Position Summary: The ED CM presents a professional image to all customers, patients, and physicians. The ED CM is accountable for assisting in transitional planning, providing a safe, supportive environment for the ED patient. The ED CM assists with community referrals as needed. Intervenes to prevent injury to patient or others according to prescribed treatment procedures, documents and reports observations to appropriate staff, makes periodic checks of patients requiring special precautions. Assists in Utilization Review activities as indicated. The ED CM provides a quality service that is focused on the utilization of resources to meet the expected outcomes of patient, family, physician, and staff. This position requires a customer service oriented individual with strong interpersonal skills for the purpose of arranging for the continuation of care and services to patients from a diverse ethnic and social group. Independent decision making is expected in the execution of all duties. Works with all insurance groups to arrange for the appropriate patient care. Strives to maintain a professional standard in case management, which includes utilizing critical thinking to assess, plan, provide, and evaluate discharge services. 3 p - 11 p Monday through Friday, and weekends and holidays as needed. Full time position. On-call after regular hours as needed to support the system needs for case management services. Qualifications: 1. Social Worker, RN or Licensed Mental Health Technician licensed to practice in the state of Kansas. 2. Three to five years hospital or comparable experience 3. Demonstrates leadership abilities; able to work under stressful situations. 4. Demonstrates interpersonal relationships and counseling skills which enhances communication, promotes conflict resolution and facilitates staff development. 5. Must have the ability to tolerate standing, walking, sitting, stooping, kneeling, pushing, and pulling 60 to 100% of time. 6. Must be able to tolerate a latex environment. 7. Computer keyboard and basic operating knowledge. 8. Demonstrates interpersonal relationships in a manner which enhances communication, promotes conflict resolution and facilities staff development. 9. Knowledge of communication skills with ability to listen both verbally and non-verbally. 10. Skill in interpersonal relationships with emphasis on caring, reaching out, being friendly and showing courtesy. 11. Skill in time management with the ability to work independently and take initiative. 12. Skill in interpersonal, oral, and written communication 13. Ability to be sensitive and show positive regard for fellow employees, patients and families. 14. Ability to take direction and work efficiently under pressure. 15. Ability to willingly accept responsibility. 16. Ability to accept direction, set priorities, and use good judgement. 17. Ability to meet organization and departmental appearance standards on the job.
