Responsibilities

Manage activities and work flow for assigned health plan clinical programs focused on improving the health status and care for individuals with chronic conditions, potentially complex medical, mental health and psychosocial issues.

• Manage all project and/or program management work including monitoring and tracking of progress and status update communications.

• Manage development and implementation of new ideas that support project work streams and clinical teams.

• Develop, implement and evaluate program services and initiatives.

• Work collaboratively with cross functional teams to promote health plan initiatives.

• Collaborate with others to assist with problem identification and resolution, cost containment issues, implementation of new services, and systems/performance measures.

• Assist with the development and monitoring of insurance policies, practices and procedures.

• Conduct hiring, training, directing, development and evaluation of staff.

Requirements

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.

• Registered Nurse credentialed from the Indiana Board of Nursing obtained prior to hire date or job transfer date required.

Education:

• Required professional licensure/certification AND 3 years of experience and 1 year of cumulative leadership experience required.

Additional Preferences

Preferred Quals: BSN and Advanced degree in nursing (MSN), healthcare administration (MHA) or business (MBA). 3-5 years of leadership or management experience. 3-5 years experience in an acute care setting; transplant experience preferred
