Description

The Manager of Inpatient Care Management is responsible for the direction and operations of the Inpatient Care Management Department. This involves supervision and evaluation of personnel, formulating and managing the department budget, policy development, regulatory compliance, quality assurance, training personnel in the responsibilities of their job functions, department optimization, and continuous quality improvement.

The Inpatient Care Management department is responsible for managing throughput of cases, matching patient needs with appropriate resources and services while optimizing length of stay, and minimizing avoidable delays in discharge. The Care Management Department institutes measures to address high-risk complex patients, improve transitions of care, improve patient outcomes, and reduce hospital readmissions. This position is integral to St.

Luke's efforts to design, develop, and implement successful care delivery methodologies needed for patient care coordination across the healthcare continuum, alternative payment models and value-based care. The Manager will work in active partnership with physicians, office staff, administration, the Outpatient Care Management Department, St.

Luke's Medical Group leadership and staff and other health care providers to develop and implement a well-coordinated plan of care to address patient needs across the continuum and ensure St.

Luke's successfully meets high quality performance metrics and optimizes resource utilization.

Principle Accountabilities: % Time Spent
1.Provides direct supervision and development to Inpatient RN and Social Work Supervisors, including: 40%

Promotes a collaborative team based environment that drives a culture of quality

Maintain department policies, practices, processes, and standards

Provides orientation and training of personnel

Responsible for hiring, firing and corrective action in accordance with St.

Luke's administrative policies

Manage staffing, scheduling, and ETO

Ensure the delivery of safe and appropriate care management and social work care through supervision, role modeling, staff education, and assurance of standard/policy compliance

Participates in the delivery of care management and social work care as needed to support staffing shortages and workload peaks, and otherwise to stay current on department processes and provide leadership by example

Provide oversight and direction to ensure achievement of performance targets and balanced scorecard objectives in accordance with the annual operating and financial plan
2.Optimize Care Management services and strategies to support the organization's strategy, vision, values and goals 25%

Ensure Care Management services support organizational goals for admission assessments, discharge planning, patient satisfaction, length-of-stay and discharge disposition Develop structures and processes that contribute to continuity of care across the continuum to reduce readmissions and manage social determinants of health

Communicate vertically and laterally to other departments as needed and keeps leadership informed of any issues

Promote a culture of two-way communication that informs, engages, and creates opportunities for staff, physicians and post-acute provider collaborative participation

Collaborate closely with the Outpatient Care Management department and Population Health department staff in accomplishing organizational goals across the continuum of care
3.Evaluate, monitor, and manage performance indicators. 20%

Regulatory ComplianceQuality AssuranceCreate and monitor quality dashboard and provide follow-up training and interventions as needed

Uses data to build and evaluate effective interventions, lead quality and performance improvement initiatives, and facilitate change 4. Develop an annual budget and assure compliance of budgetary goals. 5% 5.

Education

Bachelor's degree in nursing required.Master's degree in nursing or related field preferred.Case Management certification, ACM or CCM, preferred.

Working Conditions

Employee is required to sit continuously and walk frequently. Employee must lift/carry a light (1-20 lbs.) medium (21-50 lbs.) and heavy (50 lbs. +) load occasionally.

Employee may be required to lift over 100 lbs. with assistance. Employee is frequently required to bend, squat, crawl, kneel, climb, lift waist to overhead. Employee is occasionally required to reach forward and reach overhead. Employee may be required to do simple grasping and pushing/pulling.

Job Relationships: Interacts regularly with patients, physicians, families and other internal and external health care providers and insurance company representatives.
