Responsibilities

The RN Manager, After-Hours oversees staff and operations of the After-Hours RN team(s). This position directly supervises both Visiting After Hours RNs (VANs) and/or the After-Hours Phone Triage RNs (AHRNs). The Manager will ensure adequate staff are available and scheduled to provide phone triage support and visits for participants experiencing medical concerns during nights, holidays, and weekends. This team provides nursing support, guidance, and education to participants, caregivers, staff, and facilities after hours in accordance with InnovAge policies and protocols. This position reports to the Director of After-Hours Services and will work in collaboration with other After-Hours Managers, Center Leadership Teams, and triads to deliver exceptional patient care.

Staff Management

  • Participates in the hiring, orientation, onboarding, competency evaluation, and training of employees to ensure highly competent and qualified personnel.

  • Participates in clinical oversight of professional staff, ensuring quality processes, performance, staff education, and documentation.

  • Maintains the after-hours nurse schedule and participates in after-hours duties as needed to provide coverage and support.

  • Assures team meets all competence and regulatory requirements and participates in the development of competency measures.

  • Manages schedules and PTO requests of assigned triage RNs and VANs.

  • Monitors relevant quality dashboardsto ensure accountability of employee's performance.

Operational Management

  • Monitors and assesses program development and progression needs, with the goal of meeting participant and business needs.

  • Onboards and orients new centers to after-hours programs as directed through new acquisitions, builds, and/or identification of new needs in current InnovAge locations.

  • Monitors progression and growth of Comfort Care, After Hours, and Triage programs to ensure participant needs are met outside of center operating hours.

  • Monitors day-to-day performance and key performance indicators (KPIs) related to After Hours operations, including call volume, trends, call times, and follow-through on outcomes from calls.

  • Recognizes and reports variances (which includes quality measures less than optimal and evaluating outcomes of care) and provides resolutions in process improvement and quality improvement for the patient via multi-disciplinary team meetings or direct reporting of information to center IDTs

Collaboration

  • Works in collaboration with appropriate leadership to address unique After Hours needs across regions/location.

  • Coordinates and collaborates with administration to identify needs, create solutions, and ensure comprehensive services are provided to participants.

  • Creates, supports, and maintains a culture of professional accountability.

  • Demonstrates flexibility and assures patients receive consistent, competent, and ethical care. Addresses customer's needs, regardless of assignment, within a timely manner.

  • Supports after hours phone triages and providers direct coverage to AHRN shifts as need to ensure appropriate acute care is provided to participants and facilities timely

  • Completes participant, caregiver, and facility phone calls to assess, determine priority of care and routes accordingly

  • Routes emergent and urgent condition calls appropriately.

  • Accurately completes all required clinical documentation consistently per InnovAge standards in electronic medical record which includes completing and signing or routing participants EMR notes and verbal orders to physician

  • Communicates with center staff to ensure appropriate hand off when changing shifts and/or rotations of call. The handoff includes transferring of phones as necessary as well as reporting current condition and needs of high-risk participants for after-hours care.

  • Performs an appropriate analysis of calls and initiates appropriate intervention/triage when necessary.

REQUIRED

  • Current applicable State issued Registered Nurses License AND Compact State Licensure AND CA RN License

  • Current CPR certification or the ability to obtain CPR certification

  • 1+ year working with the frail or elderly.

  • 5+ years Nursing

  • 3+ year people leadership experience

PREFERRED

  • Bachelor's degree in nursing or higher

  • 3+ years health care experience with emphasis in geriatrics

  • Bi-lingual-Spanish, Mandarin

  • Oversight of afterhours clinical care, triage, or clinical call center experience

Posted Salary Range

USD $115,000.00 - USD $125,000.00 /Yr.
