  • Obtain and process MD orders for all referrals.

  • Using professional discretion and independent judgment, expedite requests for services appropriately adhering to criteria for admissions.

  • Collects and documents detailed, pertinent information relative to a prospective patient's health care needs utilizing knowledge of clinical standards, procedures, medications, coding standards and community health care principles.

  • Assumes intake responsibilities as per standard and procedure of the Agency.

  • Provides information and education regarding Agency services and programs to the public.

  • Works collaboratively and professionally with other members of the health care team to ensure quality of services as well as patient safety.

  • Participates in Agency, clinical, QI, liaison, clinical reimbursement and case conferences activities as required.

  • Provides clinical expertise and serves as resource to staff and community.

  • Efficiently and accurately enters data into the computer system.

  • Participates in orientation of staff to the department.

  • Maintains current knowledge of Agency policies, procedures, objectives and current documentation requirements to meet regulatory standards.

  • Maintains current knowledge of reimbursement protocols, existing health care contracts, billing protocols, and existing clinical practice guidelines.

  • Able to temporarily assume the responsibilities of the Intake Clerical Assistant and the Clinical Reimbursement Specialist when required.

  • Other duties as assigned.

  • Registered Nurse currently in licensed in Massachusetts.

  • 3 years clinical experience, with 1 year community health experience.

  • Word processing/data entry skill required.

  • BSN preferred.

Per Diem LPNs/RNs at BAMSI provide direct care service to individuals living in supervised residential settings and identify health related needs, monitor medical status and work with residential staff to ensure a safe and healthy environment for individuals served. Must be an LPN or RN in the Commonwealth of Massachusetts with at least 6 months experience in an acute care or long term care facility. A valid driver's license in state of residence is required. Responsibilities General Statement of Duties: Provide coverage for open nursing shifts in assigned residential/ day habilitation programs. Assess, plan, evaluate and implement care according to accepted standards of practice. Responsibilities: Attend to the daily needs of individuals as well as foster increased independence and self-determination. Assist individuals with personal care tasks, which include but not limited to: toileting, showering/bathing, grooming, personal hygiene, assisting with mobility, repositioning, dressing, range of motion exercises, meal preparation and eating. Follow individual protocols, treatment plans, behavior plans, etc. When indicated, follow the Department of Public Health's Medication Administration Program policies. Ensure safe and proper storage of medications; ensure medications are administered as prescribed; ensure documentation is completed as required. Review medical records and physician orders to ensure program compliance, updating information as needed. When indicated, work with community physicians and program staff to formulate and implement effective medical treatment plans. Assess, document and appropriately utilize necessary data, daily progress notes, staff log and complete all other and program required documentation. Advise program manager / nurse manager and staff of any medical problems that may hinder an individual's progress or that may be aggravated by activities at the program. Consult with staff on methods of medical treatment and care. Transport consumers to and from day programs, medical appointments and other activities as required. Provide opportunities for leisure time activities and foster independence in the choice and participation of those activities. Maintain knowledge of ongoing program issues and developments as well as informing supervisor of program issues and concerns. Provide training and orientation for direct care / nursing staff as needed. Attend required trainings. Assist with admissions and discharges to and from hospitals and rehabilitation centers. Perform other related work duties as needed or as assigned by program manager, nurse manager or designee. Perform duties to reflect agency policies and procedures. Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized and training obtained in areas that constitute risk. Ability to lift, transfer, push/pull, maneuver, and reposition 25 pounds and over. Qualifications * Possession of Massachusetts license as a nurse with experience working with disabled population. * Knowledge of the theories, methods, procedures and practices of nursing care and physical rehabilitation as applicable to community residential services. * At least six months experience in an acute care or long term care facility. * Work experience in a health or human service preferred; experience with people with developmental disabilities in a community setting desired. * Must possess a valid driver's license in state of residence and own means of transportation required. * Basic computer literacy, including the ability to record data, use electronic time and attendance software and receive and send electronic mail, as Bamsi Plymouth MA
