Responsibilities

Role:

Intake Coordinator RN is responsible for the coordination of care for referred hospice patients. Work includes assuring safe and effective transitions from hospital and community settings to hospice care. An incumbent is also responsible for symptom management, coordination of the interdisciplinary team, assessment of patient and family physical, psychosocial and spiritual needs.

Work is performed in conjunction with an interdisciplinary team and an incumbent collaborates with a variety of stakeholders in determining the hospice-related needs of the patient.

Qualifications

Education/Experience

Graduation from an accredited school of nursing, and two years of related nursing work experience in a Hospice or hospital-based palliative care setting that would demonstrate attainment of the requisite job knowledge skills/abilities.

BNS degree is preferred.

License/Certification

Current licensure as a Registered Nurse by the Washington State Board of Nursing.

Current Healthcare Provider BLS Certification.
