DEPARTMENTAL SUMMARY: St Charles Home Health Services provides skilled nursing, physical therapy, occupational therapy, speech therapy, medical social work and home health aide services to patients in their homes. Services are provided by interdisciplinary team based on patient's needs to maximize patient's outcomes and to prevent hospitalizations.
POSITION OVERVIEW: The Intake and Referral Specialist maintains daily contact with physicians, case managers, social workers, discharge planners, patients, families, caregivers, home health and hospice agencies and other referral sources in the hospital and the community regarding potential referrals to home health services.
Markets the services of the home health department. Develops relationships with physicians, providers, skilled nursing facilities, payers and others in Central Oregon. Prepares patients and families for admission to the home health department, and facilitates that process. This position does not directly manage any other caregivers
ESSENTIAL FUNCTIONS AND DUTIES:
Accepts service referrals and clarifies needed services where possible (via fax, telephone, or direct contact).
Coordinates referrals to Home Health and clarifies needed information (e.g., patient/family home health needs/expectations, diagnoses, appropriate demographic information, addresses, contact information, initial supply needs, special instructions, insurance information, etc.).
Meets directly with patients in the hospital or local skilled nursing facilities to educate on home health services and to assist with determining patients level of need.
Provides education about Home Health Services to potential referrals and referral sources.
Evaluates prospective patient situations, including homebound status, to determine if appropriate for Home Health services.
Timely documentation review to ensure accuracy of diagnosis coding, sequencing of coding, and completion of OASIS.
Communicates with patient, family, and referral sources the progress and disposition regarding referral status.
Assists with payer verification and authorization for patient care.
Knowledgable regarding the home health benefit with commonly encountered insurance companies.
Coordinate ETO and the general monthly schedule for Intake and Referral Specialists requesting coverage where needed.
Initial diagnosis review (providing a couple of pertinent "working" diagnostic codes at the time of intake or acceptance of a referral).
Identify patients on admission that may be appropriate for Lifeline Services.
Markets the services of the Home Health department directly to patients, familys, the hospital, local Skilled Nursing Facilities, physicians and other referral sources
Establish and maintain relationships with patients, referral sources and staff through consistent professional/informational communication.
Meet departmental standards and deadlines for timely completion of educational/CBL requirements (specific to department and specific to discipline).

Calculates dosages and administers medications accurately in accordance with the organization's policies and procedures for medication administration.
EDUCATION:
Required: As required for licensure.
Preferred: BSN
LICENSURE/CERTIFICATION/REGISTRATION:
Required: Current RN licensure with the Oregon State Board of Nursing
Valid Oregon driver's license and ability to meet St. Charles Health System driving requirements

EXPERIENCE:
Required: Minimum of 3 years experience as a staff nurse; one year experience in home health care or public health nursing.
Preferred: Home health experience
PERSONAL PROTECTIVE EQUIPMENT:
Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.
ADDITIONAL POSITION INFORMATION:
Position Specific:
Thorough knowledge of home health care, community resources, the insurance authorization system and nursing practice
required. Proficient in utilizing Microsoft programs and able to access a variety of EMR platforms.
Ability to follow verbal and written communications in order to complete duties and assignments

PHYSICAL REQUIREMENTS:

Frequently (51% - 75%): Standing, sitting, walking, lifting 1-10 lbs and keyboard operation.
Occasionally (26% - 50%): Bending, climbing stairs, reaching overhead, carrying 1-10 lbs, pushing/pulling 1-10 lbs and grasping/squeezing.
