ptShift Details: M-F Days; As NeededDepartment: St Joseph's Nursing | Reports To: Director of Nursing-Nursing HomeThe mission of Faith Regional Health Services is to serve Christ by providing all people with exemplary medical services in an environment of love and care.Summary:The Infection Preventionist (IP)/Educator is a registered nurse (RN) under the supervision and direction of the Director of Nursing and oversees infection prevention and education for both St. Joseph's Rehabilitation and Care Center and Skyview Villa. The RN works within the state defined scope of practice and adheres to hospital and nursing home/assisted living policies/procedures and regulatory requirements.The RN Infection Preventionist/Educator has two roles:1.The Infection Preventionist role includes organizing, coordinating, educating, and assessing the nursing home and assisted living infection control practices and works closely with the hospital infection control nurse and infectious disease provider. Provides surveillance activities as well as identification, investigation, and education on prevention. This position also oversees all aspects of infectious disease (such as Covid-19) testing, tracking, training, and result recording and reporting. This position is knowledgeable of all testing set up and procedures and ensures all regulatory requirements are met and maintained.2.The Educator role includes being responsible for assessing, planning, developing, and evaluating orientation, competencies, in-service education, and continuing education that contributes to enhancing employee performance and organizational success.The listing of job duties contained in this job description is not all inclusive.   • Conducts individualized patient assessment, prioritizing the data collection based on the patient's immediate condition or needs within the timeframe specified by policy. * Conducts ongoing assessments as determined by patient condition and policy and reprioritizes care accordingly. * Develops and updates the plan of care throughout the patient stay ensuring that is individualized and reflects collaboration with other health care team members. * Identifies patient/family discharge planning needs upon admission. * Performs appropriate treatments including medication administration per physician order and per policy in an accurate and timely manner. * Reports patient concerns and condition changes to the provider in a timely manner. * Performs therapeutic nursing interventions in line with the plan of care. * Provides individualized patient/family education customized to patient and family needs. * Documents all nursing cares and education accurately and timely in the electronic health record. * Documents response to nursing cares and education accurately and timely in the electronic health record. * Delegates and supervises appropriate patient care. * Provides care in a non-judgmental, non-discriminatory manner that is sensitive to patient and family diversity, preserving autonomy, dignity, and rights. * Actively participates and involves the patient and family in handoffs and bedside shift report and relates pertinent patient information. * Responds appropriately, professionally, with composure and confidence during emergencies and stressful situations.3.Performs all responsibilities/duties of the Infection Preventionist and Educator. Department and/or Role Specific duties include:*
  • Infection Preventionist:*
  • Evaluates infection control practices, as well as resident care products, equipment and supplies and ensures that appropriate policies are developed and instruction for use are followed. * Works in collaboration with the RN Educator to develop and implement educational sessions for orientation and ongoing education regarding infection control (IC) practices. * Develops and reviews policies and procedures related to IC. * Works collaboratively with the interdisciplinary team to complete risk assessments identifying gaps and needed interventions. Develops and monitors appropriate interventions for compliance. * Provides guidelines to physicians, nursing, and other personnel in determining appropriate actions when dealing with infections and their treatment and obtains assistance from infectious disease consultants as necessary. * Reviews best practices and regulatory agency requirements and/or recommendations and updates the IC plan as needed. * Monitors facility environment and personnel practices, i.e. isolation practices, disinfection, and staff using appropriate personal protective equipment. * Maintains accurate organized records that are readily accessible. * Submits any required reporting to internal and external departments/agencies in an accurate and timely manner. * Compiles, organizes, and analyzes statistical data for reports and performance improvement activities. * Oversees all aspects of infectious disease testing requirements: * Testing Site * Testing, tracking, reporting, and training * Maintains accurate logs * Educator:*
  • Responsible for orientation of all staff in all departments at the facility. Also responsible for on-going training and education as needed. * Assists with and/or develops organizational education to include nursing orientation, annual competency based educational programs, and mandated programs. * Assesses department needs & communicates to administrator/DON for training and responds accordingly. * Schedules training programs and in-services for all employees. * Maintains appropriate documentation of orientation and in-service training for employees. * Assists with development of and educates staff on new policies and procedures. * Follows up with new employees at regular intervals to determine need for additional training and support. * Lectures and demonstrates procedures, using motion picture, DVD's, videos, charts, or other methods; uses outside consultants/speakers when needed. * Observes employees in practical application of procedures and does one-to-one training as needed. * Supervises use of company provided online education and monitors compliance. * Assists with developing recruitment strategies for positions. * Provides input on educational budgetary needs. * Monitors safe working practices; provides education and coordinates with Safety Committee for corrective/preventive actions. * Assists with the Continuous Quality Improvement Program, Safety Committee, and other committees as needed. * Ensure orientation to Electronic Medical Records system with periodic review for understanding of proper documentation. * Assists with audits as directed and evaluate educational needs related to audits. * Stays current with evidence-based practice related to regulatory/governing bodies. * Maintains effective communication with residents, families, and facility staff.4.Assumes all other duties and responsibilities as necessary.*
  • Maintains confidentiality per policy. * Maintains current competency by attending unit and hospital based educational offerings either in person or online. * Actively engages in unit activities and team meetings and attends required meetings. * Demonstrates active engagement in department activities and evidence-based initiatives. * Demonstrates dependability/flexibility in meeting scheduling needs of the department and hospital. * Demonstrates willingness to float to other units as needed. * Accepts assignment of preceptor for new staff. * Accepts responsibility for mentoring healthcare students. Ensures the quality and safety of the patients and staff if serving in a relief charge nurse role.Hours will be dependent on patient census and workload.   • Current Registered Nurse (RN) license or temporary permit from the State of Nebraska required. * Basic Life Support (BLS) obtained within one full month from date of hire required. * Nursing Home Infection Preventionist Training Course completed within one year from date of hire required.Previous Experience Requirements:EXPERIENCE:Previous clinical experience preferred.Skills/Knowledge Requirements:SKILLS:*
