SUMMARY OF JOB RESPONSIBILITIES:

Provide leadership, administration and support in assigned area(s). To ensure achievement of Hospice of the Valley's strategic and operational plans by effectively managing human, financial and capital resources within the spirit of its mission and values. Assumes responsibility for coordinating all available resources to assure safe, appropriate care and treatment for all patients/families needing hospice services; acts as practitioner, role model, educator and change agent within their team. Works collaboratively to plan implement and evaluate practice standards within the team/agency. Works with the director to evaluate practice standards and provide problem resolution using the interdisciplinary team approach.

QUALIFICATIONS:

Certification in Hospice and Palliative Care expected within 2 years of appointment to position.

Maintains current CPR certification.

Knowledge related to electronic medical records.

EDUCATION:

Graduate of an accredited school of nursing and current RN licensure in the State of Ohio. BS/BSN required or attainable within two years of appointment to position(waived for incumbants).

Has in-depth knowledge of the hospice/palliative philosphy of care.

Analyzes and interprets complicated information.

Determines a course of action based on general guidelines.

Incorporates understanding of regulations/protocols within area of responsibility.

PRINCIPAL JOB ACCOUNTABILITIES:

Responsible for facilitating and coordinating referrals and admissions to Hospice of the Valley.

Fosters the spirit of servant-leadership by promoting empowerment, collaboration, performance improvement processes, inter-disciplinary work teams and partnership relationships with customers. Maintains current knowledge of Medicare Conditions of Participation for Hospice Care. Creates an environment which fosters professional/personal growth and development. Provides orientation, in-service training and continuing education of staff. Participates in QAPI (Quality Assurance and Performance Improvement) for the agency. Participates in data collection and review of statistics Maintains open and effective communication with Medical Director(s) and other members of the interdisciplinary team in reviewing referred cases. Promotes stewardship of resources while maintaining or improving quality of care. Exhibits and encourages ownership for the success of the organization. Regularly communicates goals, plans and actions encouraging employee input and ownership. Effectively engages staff in two-way dialogue and problem solving.

SUMMARY OF JOB RESPONSIBILITIES The RN provides skilled professional nursing care to the patient/family/caregiver as prescribed by the physician and in compliance with regulations as established by the State Board of Registered Nursing, the patient, and the policies of Hospice of the Valley. QUALIFICATIONS: Is a graduate of an accredited school of professional nursing. BSN preferred. Is currently licensed as an RN through the Ohio Board of Nursing. At least two year's general nursing experience is required with current hospice, medical, surgical or critical care experience. Has experience with providing good pain management, IV skills preferred. Previous home care experience preferred. Complies with accepted professional standards and principles. Understands hospice philosophy, principles of death/dying. Possesses and maintains good physical stamina and mental health. Has a valid driver's license and is insured in accordance with state and/or organizational requirements. Possesses and maintains current CPR certification. Has excellent observation, good nursing judgment and communication skills. Ability to provide good written documentation in a timely manner. Is self-directed with the ability to work with little supervision. Is flexible and cooperative in fulfilling role obligation and has ability to work within an interdisciplinary team. RESPONSIBILITIES AND DUTIES Seeks and obtains sufficient information necessary to function effectively in the hospice setting where patient/family/caregiver are requiring care. Provides professional nursing care by utilizing all elements of nursing process. Assesses and evaluates patient's status by: Writing and initiating plan of care Regularly re-evaluating patient/family/caregiver needs Revising the plan of care as necessary Initiates and sustains as needed the implementation of orders for medications and treatments as prescribed by the physician in the physician's plan of care. Initiates and applies appropriate preventative and therapeutic nursing procedures and techniques and maintains communication regarding patient care with the physician and supervisory personnel. Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Uses supplies and equipment effectively and efficiently. Observes the patient for changes in condition and reports these changes and the patient's/family's/caregiver's needs to the attending physician and appropriate supervisory personnel. Records pain/symptom management changes/outcomes as appropriate. Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optimal comfort care. Teaches the patient/family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients/families/caregivers. Works in concert with the other Hospice interdisciplinary team members. Provides and maintains a safe environment for the patient. Supervises ancillary personnel and delegates responsibilities when required. Plans for patient discharge from service as appropriate. Accepts responsibility of assignment to perform a specialized procedure, such as IV Therapy, only when qualified with specialty training, proven competency, clinical background, and expertise in the specialty area. Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes. Evaluates own need for support and uses identified system to meet these needs. Fulfills the obligation of requested and/or accepted case assignments. Actively participates in quality management program. Accepts on call duties and team leader responsibilities as needed. Performs other duties as assigned.
