Overview

Role:

The Hospice House RN is responsible for assessing and identifying patient/family needs, utilizing the nursing process; coordinating the plan of care with the Interdisciplinary Group (IDG) and providing palliative and supportive care to the patient/family unit. The RN utilizes nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit during times of crisis.

In addition to RN Hospice House responsibilities, the RN Hospice House Preceptor plans, coordinates and carries out educational opportunities for Hospice House nurses. S/he orients the nurses to their role within the interdisciplinary team; evaluates job performance using competency checklist to insure each new nurse has successfully acquired knowledge and skills essential to providing patient care.

Qualifications:

  • Current Florida license as RN, or current licensure from another state plus a temporary Florida permit

  • Valid driver's license and automobile insurance per Company policy

  • Reliable transportation to meet visit schedule

  • Minimum one (1) year in-patient nursing or hospice experience

  • Ability to use equipment with visual and auditory mechanisms.

  • Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system required

  • Ability to effectively communicate in English

  • Flexible schedule to meet the needs of Hospice House patients

  • Ability to manage the emotional stress of working with and caring for terminally ill patients and their families

  • Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation

  • Ability to physically access any home i.e. navigate stairs and narrow spaces, tolerate heat and lack of air conditioning, in any area Company serves.

Ability to physically access patient rooms and other areas of the Hospice House

  • RN Preceptor:

  • Minimum two (2) years' experience in the role of Hospice RN with a minimum tenure at a Chapters' affiliate Hospice House of one (1) year

  • Satisfactorily complete competency requirements for this position.

Job Responsibilities:

Clinical

  • Act as primary clinical resource during shift for clinical staff and IDG.

  • Enhance the clinical practice of IDG staff by performing as a role model, mentor, instructor and resource person.

  • Provide coverage/assistance to staff as needed and as patient acuity dictates to ensure continuity of care to all patients.

  • Coordinate patient care, including assessing psychosocial, spiritual care, and volunteer needs, and make appropriate referrals, as needed.

  • Provide and manage direct care to patients and families as part of Interdisciplinary Group (IDG), incorporating psychosocial, spiritual, cultural, physical and biological components, and appropriate nursing intervention and follow-up.

  • Coordinate the plan of care, insuring that an individualized plan of care is developed that accurately reflects the patient's evolving needs.

  • Demonstrate clinical skills and provide education to shift staff regarding: case management, application of company policy and procedure, physical assessment, intravenous therapies, pain medication titration, symptom management, and patient/family relationships.

  • Consult, refer, and collaborate with other professionals to promote quality and continuity of patient/family care.

  • Take verbal orders from physicians, ARNPs and physician's assistants.

  • Educate patient, family, caregivers and other health professionals about disease process and decline, palliative interventions, care giving, dying process and safety practices.

  • Report changes in the patient's condition to appropriate members of the IDG or other health professionals.

  • Participate with the IDG to evaluate hospice house referrals/admissions for level of care appropriateness.

  • Attend patient deaths; prepare body for transport; oversee disposal of medications.

  • Provide appropriate bereavement interventions to the family and caregiver in conjunction with psychosocial staff.

  • Accept responsibility and is accountable for nursing actions: demonstrate an understanding of the legal consequences of nursing actions; request supervision when needed; perform within the guidelines established by the Florida Nurse Practice Act.Administrative

  • Document assessment, plan, intervention and evaluation on appropriate Company forms with accuracy, timeliness, legibility, professional terminology and awareness of legal and regulatory implications.

  • Document patient condition and decline to support hospice and level of care appropriateness.

  • Present concise and pertinent oral and written reports to IDG; respect and encourage input from all disciplines.

  • Communicate accurately and completely to physicians, staff members, patients, families, and supervisors; utilize positive approaches when working with others.

  • Supervise patient care given by other nursing team members; i.e., LPNs, hospice aides, and contract nurses.

  • Exemplify behaviors supportive of internal mission statement.

  • Participate in orienting new staff, students, volunteers and board members.

  • Participate on committees, projects and department planning, as assigned.

  • Act as a resource to peers, students, orientees and other staff members during shift.

  • Participate in activities and professional development that enhance hospice nursing skills.

  • Participate in the broad education process of the Company, both to the medical and lay communities.

  • Collaborate with other departments (i.e., Medical Services, Pharmacy, Infusion Program, and Continuous Care) to ensure patient care needs are addressed.

  • Supervise patient care given by other nursing team members; i.e., LPNs, hospice aides, and contract nurses.

  • Provide written/verbal input to the evaluation of IDG staff to the Director or Manager.

  • Communication

  • Communicate in a clear and accurate manner. Provide constructive feedback and resolve conflict directly. Assist others to improve communication skills.

  • Incorporate joint decision-making and unified goal achievement into IDG practice.

  • Demonstrate respect and understanding for all clinical disciplines required for quality patient care and mentor staff in this process.

    Responsibilities:
  • Plan, coordinate, and carry out educational opportunities for the nurses.

  • Explain the role and performance expectation for the Hospice House nurse in the hospice setting.

  • Assist the orientee to identify learning needs.

  • Plan for and provide relevant learning experiences involving other staff as necessary.

  • Demonstrate and/or supervise nursing procedures or skills that are unfamiliar to the orientee.

  • Orient new nurses to their roles within the IDG and hospice house unit.

  • Acquaint the orientee with the concept of IDG through participation in IDG meeting.

  • Evaluate competence of orientee utilizing orientation competency checklist.

  • Provide prompt and consistent feedback, identifying areas in need of further development to the new nurse and the Manager.

  • Coordinate activities with other shift nurses to maintain consistency in practice, establish relationships, and broaden the scope of the program.

  • Provide input to clinical leadership and educators regarding patient care experiences for orientation.

  • Participate in Nurse Preceptor Meetings.
