Summary

The Care Coordinator will coordinate a panel of patients throughout the continuum of care to assure that care is timely, appropriate, of high quality and cost effective.
Responsibilities

The RN will have a panel of 70 patients. Patients will be enrolled into the HT Program for varying DMPs such as COPD, HF, PTSD, HTN, etc.

The RN will monitor the daily responses of the patients on his/her panel and will provide an intensive care management and education to the Veterans. The RN willwork closely with the primary care providers(s) and other healthcare professionals and team members, other clinics internal or external services and community agencies. Other responsibilities include but are not limited to: patient assessment of needs and resources for case management; applies the nursing process for desired outcomes for discharge, post hospitalization recovery and health maintenance/improvement; collaborates with patient, providers and family members in all settings to develop and evaluate plan of care and goals; etc.

Home Telehealth training for procedures and equipment required within one month of assignment to position. The Care Coordinator will utilize clinical video telehealth modality to perform live synchronous healthcare encounters with patients on their panel. Additionally, the care coordinator will participate at PACT meetings and will provide education and support for the expansion of the HT program.

The incumbent will be expected to data mine and scrub charts with the scope of identifying patients that are eligible for the HT program and will place consults for the program in the electronic patient record. The incumbent will be expected to float to other units when staffing needs fluctuate.

Work Schedule: 8 hour shifts. This will include rotating days off, rotating tours of duty, and weekend and holiday coverage. The shifts are also subject to change.

Qualifications

Basic Requirements:

United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing or The Commission on Collegiate Nursing Education . In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Education & Experience:
MSN
Certified Diabetic Educator and Certified in Case Management.
Minimum of 1-2 years medical/surgical experience.
Working knowledge of the application of the clinical expectations in the management of geriatric patients.
Responds to patient/significant other, visitor, and/or staff needs in a courteous manner.
Demonstrated ability to perform duties in accordance with the Joint Commission Standards and the Nursing Standards of Care.
Ability to easily establish and maintain good interpersonal relationships and communication with physicians, staff, patients, and significant others.
Ability to communicate effectively both verbally and in writing.
Basic computer knowledge and skills. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree.
If a current VA employee is selected, he/she will be reassigned at their current grade and step. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office.

Physical Requirements:
Near vision correctable at 13" to 16"; far vision correctable in one eye to 20/25 or better.

Ability to distinguish shades of color is not required but would be beneficial. Incumbent will sit, type, walk and talk with staff, manage databases. S/He should be able to carry 20 lbs for long distances in extreme heat on an intermittent basis.

The physical requirements of this position include lifting, prolonged standing and sitting, frequent bending, walking (distance), climbing (steps, ladder, reaching (overhead, extensive/repetitive), repetitive motion and stooping. The mental/sensory requirements include recall, reasoning, problem-solving, hearing, speaking clearly, writing legibly, keyboarding, reading and logical thinking. The environment's pace can vary from steady to a sometimes fast pace requiring handling of multiple priorities, frequent, sometimes intense customer interactions, and the ability to adapt to frequent changes in a sometimes noisy environment. There may be the need to use personal protective equipment to prevent exposure to disease, illness and hazardous materials.

Annual leave shall accrue for full-time Nurses, and Nurse Anesthetists at the rate of 8 hours for each full biweekly pay period.
