Purpose: We are seeking a dedicated and compassionate Registered Nurse to join our Home Health team. As an RN Home Health, you will provide skilled nursing care to patients in their place of residence on weekends. This includes administering medications, performing assessments, implementing care plans, and collaborating with the interdisciplinary care team.

Responsibilities:

• Conduct initial evaluation visits and assess patient needs
• Develop and implement comprehensive care plans
• Administer medication and treatments as prescribed by the physician
• Monitor patients' progress and update care plans accordingly
• Educate patients and their families on self-care techniques and discharge instructions
• Coordinate with other healthcare professionals such as physical therapists, occupational therapists, and social workers
• Ensure compliance with regulatory requirements and agency policies

Requirements:

• Graduate of an accredited nursing program and current RN license
• Minimum of 1 year of experience in home health or a related field
• Excellent clinical assessment and critical thinking skills
• Strong communication and interpersonal skills
• Ability to work independently and manage time effectively
• Valid driver's license and reliable transportation
• CPR certification
