We are seeking a Care Manager for one of our Internal Medicine Offices, located at 400 Highland Ave.Salem, MA 01970.
The RN High Risk Care Manager, as a member of the health care team, collaborates to enhance the delivery of patient care services along the continuum of care. Utilizing a patient centered approach; the care manger directly interfaces with physicians, health care teams, patients and their care givers in managing their care.
The RN High Risk Care Manager meets patient's needs efficiently and expeditiously by continuously working to improve the patient's experience, helping to ensure high quality patient care and reducing cost. Through broad knowledge of clinical care and systems management, the RN High Risk Care Manager evaluates, predicts, and facilitates the trajectory of patient care. Required

Completion of an accredited Nursing degree program.
Minimum five years nursing experience.
Registered Nurse with current license to practice in Massachusetts.
Acute hospital experience preferred.
Minimum of 2 years acute care experience. Other experience may include post acute care settings or care coordination/case management experience.
Bachelor's Degree preferred.
