Job Description

JOB SUMMARY:

The RN Healthy Beginnings is a professional caregiver who is a member of the interdisciplinary team and is responsible for providing comprehensive nursing services to women and their families eligible for the Nurse-Family Partnership Program(NFP). Nurse home visitors are responsible for maintaining the highest standards in clinical nursing practice and adherence to the NFP model, and to policies, procedures, guidelines, and standards of NFP and of the Carle Health System. Additional responsibilities include the coordination and provision of direct and indirect patient care using the nursing process to meet the physical, psychosocial, environmental, and spiritual needs of Healthy Beginnings specific patient populations and families throughout the geographical area.

Educational Requirements: Associate Degree in Nursing, Bachelor's Degree in Nursing required within 5 years of hire date.

Licensure/Certification Requirements: Illinois RN, BLS (within 30 days), Lactation Consultant Certification (within 1 year), Driver's License and Proof of Auto Insurance.

Experience Requirement: Two (2) years of experience in public/community health, home health, case management, mental/behavioral health, pediatric/child, family medicine, woman's health, other experience will be taken into consideration. Experience with diverse populations, communities, & home visits preferred.

Specialized Knowledge and Skills Requirements: Organized, responsive, and dependable. Culturally competent and caring demeanor. Strong communication (written and verbal). Is creative, innovative, problem-solver and team -centered. Must be able to work independently as well as function as part of the home visiting team. Has strong knowledge of prenatal, early childhood, or disadvantaged populations. Attends and participates in bi-weekly case conferences, team meetings, as well as weekly nurse supervisor meetings for clinical supervision in accordance with NFP requirements; Creates a culture of caring, trust, as well as support and growth with families, community members, and team members; Actively engages in professional development to meet all NFP Nursing Home Visitor competency requirements; Engages in continuous, organizational learning. As new knowledge is acquired through evaluation, study, experience, and innovative approaches, processes and initiatives are refined; Develops and maintains knowledge base of current and applicable federal and state laws, regulations, and accreditation standards; Demonstrates a deep understanding and respect for the community.

ESSENTIAL FUNCTIONS:

  • Provides home visits to women and their families eligible for the NFP Program.

  • Coordinates the care/services for each client; carries a case load of up to 25 clients.

  • As requested/required, participates in community coalition in support of NFP as well as promotes public awareness of NFP and the Healthy Beginnings program.

  • Develops and maintains community relationships to support client referrals.

  • Follows nursing process in implementing NFP into nursing practice; Develops therapeutic relationships, utilize concepts of reflection and motivational interviewing with women and their families in a home visiting environment.

  • Assesses physical, emotional, social, spiritual, and environmental needs of women and their families as they relate to the NFP domains.

  • Assists women and their families in establishing goals and outcomes; Provides education, support and referral resources in assisting women and their families in attaining their targeted goals

  • Participates in review and analysis of reports for achievements and areas for improvement as well as quality improvement efforts.

  • Formulates nursing diagnosis based on nursing assessment and client goals and then evaluates client progress toward stated goals and NFP outcomes.

  • Plans home visits as well as joint home visits with Healthy Beginnings Nurse Supervisor in accordance with client goal and NFP outcomes and expectations.

  • Consults and collaborates with other professionals involved in providing services to women and families.

  • Understands, supports, represents, and coaches others in the NFP vision, mission, and model in actions and verbally to both internal and external customers and colleagues.
