Provide and coordinate care by assessing physical and behavioral health needs of patient, develop and implement nursing care plans, maintain medical records and educate patients and their families about various physical and behavioral health conditions.

Provide nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures and protocols, as well as facility and corporate policies and procedures.

Ensure physician's orders are legible if taken verbally and transcribed appropriately per policy and procedure.

Ensure patient's status is assessed on an ongoing basis and pertinent information gathered is documented. Notify physician of significant changes in status such as difficulties with detox, medical emergency or change in medical status regarding response to medications, etc.

Nursing treatment plans are specific to assessed needs. Initial treatment plan is completed upon admission.

Demonstrate knowledge and utilization of universal precautions in providing direct patient care.

Accurately administer medications per ordered note and document accurate count of controlled substance medication administration on required documents.

Responsible for completing admission and discharge processed efficiently and accurately as directed.

Exhibit therapeutic rapport with patients by maintaining professional boundaries, being respectful to personal space, giving patient direct attention and maintaining rational detachment.

Provide direction or coordinate the activities of the unit as directed.

Communicate therapeutically and professionally with patients, visitors, coworkers and others involved in patient treatment in an effective manner.


Perform other functions and tasks as assigned.


Graduation from an accredited school of nursing.


Current RN license in State of Texas or Multistate License. CPR and CPI certification required (training available upon hire and offered by facility).

The RN II works under the direct supervision of the Assistant Chief Nurse Executive to provide routine care and treatment through the application of the nursing process. Is responsible for maintaining nursing standards of practice in accordance with the Nurse Practice Act, HHSC Policies and Procedures, the Texas Administrative Codes and other facility policies, procedures and standards. Communicates and collaborates with other shift RN's to ensure 24-hour continuity of care. Ensures adequate staffing according to the program specific staffing plan; determines staff assignments based of appropriate knowledge and skill to meet individual patient needs. Models and supports a proactive approach to unit management using the principles of Social Learning and recovery. Demonstrates a commitment to a culture that minimizes the use of restrictive interventions. Provides direct supervision to all Psychiatric Nursing Assistants and Licensed Vocational Nurses. Must understand that duties, location of work and shift patterns are subject to change, as determined by the administration, to meet the needs of the hospital. This employee has full access to a limited set of records in accordance with HIPAA regulations.Essential Job Functions: Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. 1.Performs extensive initial and on-going assessment of the patient's nursing care needs to include medical, psychological, and social needs. 2.Collaborates with other health care professionals by contributing nursing assessment data, diagnoses/problems, towards the development of a multidisciplinary recovery plan. 3.Evaluates the outcomes of nursing interventions and documents the same in the medical record. 4.Provides comprehensive patient/family education regarding illness process, medications, coping skills, etc. to patients and families. 5.Establishes, monitors, and maintains the therapeutic milieu through clinical supervision and training of other Registered Nurses, Licensed Vocational Nurses, and Psychiatric Nursing Assistants. 6.Supervises the administration of medications, and other nursing interventions in accordance with facility policy and procedures. 7.Acts as team leader in emergency situations (i.e. medical or psychiatric emergencies) including initiating and monitoring the appropriate use of resuscitative equipment or interventions such as restraint/seclusion.8.As assigned, serves as the contact RN. 9.Employee actively participates and /or serves in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.
