Job Title: RN PACE
Job Summary
Under the general supervision of the clinic manager RN and medical director of FLORIDA PACE CENTERS, provides nursing care to all persons enrolled in the program.
Essential Job Functions
Performs comprehensive nursing history and physical on new referrals.
Creates appropriate care plan and participates in IDT.
Evaluates and triages participants with episodic illnesses
Refers participants to medical director or medical specialist as indicated
Actively participates in family conferences.
Participates in interdisciplinary team meetings, committees and work groups as assigned.
Job Requirements
Education
Experience:
At least 1 year of RN or 3 years of LPN required
Minimum one-year experience working with a frail/elderly population
Licenses/Certifications:
Current RN license by the Florida State Board of Nursing
Valid Florida Driver's License & Proof of Insurance
BLS certification required
ACLS preferred
Abilities Required
Bilingual English/Spanish, knowledge of Creole a plus
Proven clinical leadership
Ability to get along with others in an interdisciplinary team environment

Normal working condition with adequate lighting and ventilation
Ability to assist transferring patients and ambulation which may involve lifting, pushing, and pulling of patients from 50-150 pounds
