Responsibilities

Renders professional nursing service to residents within an assigned unit/wing in support of medical care as directed by the medical staff; supervises CNAs to ensure a high quality of care is provided to all residents of that unit.

Responsible for daily nursing operations of a specific unit/wing.Performs nursing techniques for the comfort and well-being of the residents; checks each resident daily to determine status; ensures residents reach highest attainable level of functioning.Prepares equipment and assists physicians during their rounds.Administers prescribed medications, orally and by injection; provides treatments using therapeutic equipment; ensures proper handling and storage of medications and controlled substances.Observes resident's reactions to medications and treatments.Observes, records, and reports changes in residents' condition to supervisor and attending physician.Maintains resident's medical record by way of documentation as required by policy.Provides education to residents and families regarding care.Assists in planning, supervising and instructing CNA; assigns daily work assignments to ensure positive outcomes for residents.Implements and maintains policies and procedures.Ensures that safe work practices are utilized.Ensures proper infection control techniques are utilized.Assists in the Continuous Quality Improvement Program and ensures follow-up occurs.Reports accurately to oncoming shift Charge Nurse regarding change in resident status and follow-up needs.Completes performance evaluations timely for CNAsCoaches and disciplines CNAs as needed - ensures personnel policies are followed.Recommends staffing, equipment and supply needs to DON.Responsible for capturing charges for all supplies issued to residents on that unit.Participates in care planning process and ensures the Nursing component is implemented.Ensures the rights of residents are respected; coordinates with other departments and ensures residents are taken to activities, etc.Other duties as assigned by supervisor.Maintains effective communication with residents, families and facility staff.QualificationsHas a current license by the State Board of Nursing as an RN.Must have the ability to lead and motivate subordinates.Ability to perform a variety of duties characterized by frequent change, as work schedule will change on a daily basis.Able to work intimately with residents, physicians, and staff.Experience in the long term care setting preferred.Capable of working to prescribed nursing standards.Good interpersonal and communication skills; has an interest to perform a service.

Participates in various other assignments as requested by the physician and clinic administrator. Coordinates patient care including patient/family education to insure quality, effective utilization of resources consistent with the hospital mission and Collaborative Patient Care Management goals and priorities. Directs the daily and short-range goal setting and planning for the assigned case type and Group Practice. Participates in the annual goal setting and planning for Group Practice. Guides health care team members in designing collaborative teaching plans/programs and in planning/providing patient/family education and discharge planning within designated specialty. Escorts patients to the exam rooms to collect vital signs, temperature, weight and other health maintenance data. Draws blood for various lab procedures. Performs EKGs and x-rays as ordered by the physician. Gives injections and prepares patients for examinations such as pap smears. Assists with minor surgery and laceration repairs. Records and reviews all prescribed medications on each patient visit. Assists in administering prescribed medications and fulfilling prescription refill requests electronically when permitted by license. Maintains open communication with nurses, physician, and supervisor. Performs all skills on competency skill checklist and attends all updates and in-service education sessions. Initiates, maintains, and performs patient education. Attains all clinical documentation for referrals, prior authorizations, etc. ensuring continuity of care for patients. Completes all required documentation in electronic medical record to meet Meaningful Use guidelines. Receives clinical messages and responds to patients needs, escalates messages to physicians attention when appropriate. Assists primary physician with all relevant aspects of patient care and fills in helping other providers and performing front office duties when necessary. Screens patient calls to physicians; handling all calls that do not require a physician. Call patients to relay advice and instructions from the physician. Keeps exam rooms clean, well stocked and maintained, properly disposing of contaminated and disposable items. Ensures instruments, supplies and equipment are ready for use. Assists and takes initiative in handling issues of a non-routine nature and performs other duties as assigned. MINIMUM QUALIFICATIONS REQUIRED: Registered Nurse with current Alabama license. Baccalaureate degree required. Two or more years of physician office experience preferred but not required. Current CPR Certification required, ACLS preferred. Demonstrates competency in assessing, planning, implementing, and evaluating care including patient/family education for patients. Demonstrates ability to facilitate team or group activities, knowledge of performance assessment. . Demonstrates leadership qualities including professional verbal and written communication skills, ability to be flexible and to prioritize in complex situations, decision-making skills, and professional development through participation in continuing education and professional organizations. Exellent verbal and written communication required. Good telephone etiquette and customer relations skills. CPT and ICD-Coding, computer literate.
