Overview

The Oncology RN Discharge Coordinator, is a disease group specific expert who provides clinical review of cancer referrals to promptly assist with scheduling of patients.

Ensures all necessary medical records are available in order to facilitate a timely and comprehensive consult visit.

Primary responsibilities include: Synthesis of new patient medical records in order to confirm appropriate scheduling of provider visits, facilitates receipt of additional records as needed, provides real time support to specialty referral coordinators to assist with scheduling, acts as primary clinical contact for patients with questions ahead of consult visit, addresses real or perceived barriers to care identified during initial phone consult, collaborates with the multi-disciplinary team, provides additional resource referral as needed, and confirms visit readiness.

Preferred Experience / Certification:

  • 3 years within oncology, with at least 1 year in assigned disease group, OCN, AOCN, APPN, CBCN, or BMTCN certification preferred, required within 24 months.

  • Must complete a minimum of 12 CEUs annually with a focus on disease group specialty if certification is not already disease group specific.

  • Participates in a department level quality improvement project annually.

RN Discharge Coordinator, PRN (24 hours per month required)

Schedule: 7am-5:30pm

Emory University Hospital Campus, Clifton Rd.

JOB DESCRIPTION:

  • Contributes to excellence in patient/family centered care by coordinating the health care of select patient populations/designated services across the continuum of care using the nursing process. Promotes the safety and well being of all patients by coordinating the patient's discharge from the hospital and transition of care to the out-patient clinic.

    Collaborates with the inpatient interdisciplinary team to develop and maintain the plan of care. Works in collaboration with others to ensure that all discharge needs are met. Schedules all discharge appointments, treatments, and protocol specific orders.

    Completes needed discharge follow up phone calls to patients and documents appropriately. Performs other duties as required. Works with manager to formulate plan for professional development.

    Attends educational in-services as appropriate. Participates in professional activities and organizations to maintain knowledge of current trends, practices, and developments. MINIMUM QUALIFICATIONS:

  • Bachelors degree in Nursing.

  • Must have a valid, active unencumbered Nursing license or temporary permit approved by the Georgia Licensing Board.

  • Three years of clinical nursing experience as a Registered Nurse with at least two years of recent experience (within the last five years) in the area of concentration.

  • OCN certification preferred for some positions.

  • BLS required; ACLS Certification preferred but may be required in some areas.
