Position Summary

City of Hope is seeking a dedicated and experienced Diabetes Educator Nurse to join our endocrinology service team. The successful candidate will play a crucial role in providing comprehensive diabetes education and facilitating care coordination for patients with diabetes. This position involves direct patient care, educational program development, and collaboration with the endocrinology team to ensure high-quality patient outcomes. This opportunity is for full-time, 8-hour shifts, and 40 hours per week. The clinic is open Monday - Friday from 8:00am to 5:30pm.

Up to $20,000 sign-on bonus available!

As a successful candidate, you will:

  • Patient Education: Provide individualized diabetes education to patients and their families, including instruction on blood glucose monitoring, medication management, insulin administration, diet, and lifestyle modifications.

  • Care Coordination: Collaborate with endocrinologists, primary care providers, dietitians, and other healthcare professionals to develop and implement comprehensive care plans for patients with diabetes.

  • Clinical Support: Conduct patient assessments, review medical histories, and monitor patient progress. Adjust care plans as needed based on patient feedback and clinical outcomes.

  • Program Development: Develop, implement, and evaluate diabetes education programs and materials. Stay updated with the latest diabetes management practices and integrate them into educational programs.

  • Patient Advocacy: Advocate for patients' needs within the healthcare system. Assist patients in navigating healthcare services and accessing resources.

  • Community Outreach: Participate in community health events and workshops to promote diabetes awareness and education.

  • Documentation: Maintain accurate and up-to-date patient records, including education provided, patient progress, and care plans. Ensure compliance with clinic policies and regulatory requirements.

Your qualifications should include:

  • Associate degree in nursing

  • To be considered for RN III - 3+ years of experience as a RN

  • To be considered for RN IV - 3+ years of experience as a RN, Current National certification, must be maintained in order to retain RN IV status, Pay Range for RN IV - $56.06-$84.09

  • Current California Registered Nurse license

  • Current BLS through American Heart Association

  • CDCES Preferred
