Position will be part of a dedicated call team for the hours of


0700-0700 Friday am-Monday am to cover the Cardiac Cath Lab for Spartanburg Medical Center.# If needed, this staff may be utilized at Pelham Medical Center Campus for cath procedures until 1200 on Friday.


Incumbent will be guaranteed a minimum of


36 hours per week.# The timekeeper will need to ensure that the associate gets the 72 hours in Kronos each pay period.# This is a unique position/situation that we have piloted only at the Pelham Medical Center in the OR. ######## Job description will read the same as the current Cath Lab positions.# These positions will require that the associate be an experienced Cath Lab associate with documented clinical and behavioral competencies.# RNs will be required to be able to function in all three roles in the lab, circulate, record and scrub.# Associates will need to be aware that this is the piloting of a new position.# If the pilot fails for any reason, this staff would fold into the current Cath Lab staff and work Monday # Friday and take

Help manage the patient#s care to increase the efficiency and quality of care that is delivered to the patient. This role will start with the time of the patient#s diagnosis and will continue through the patient#s journey throughout their care. Will be the patient advocate and care for them in a highly supportive and personal manner and also serve as a liaison to ensure the patient#s and family#s receive resources and services as needed. Functions to include: Provide patient education pre/peri/Post procedure regarding diagnosis. Contacts every newly diagnosed patient at the time of diagnosis/referral. Ensures and coordinates new patient referrals, scheduling of exams, procedures and appointments, identifies potential gaps as well as the coordination of care for patients returning to their communities (outside of MGC network) for treatment and follow-Uup. Frailty/KCCQ12 for procedure patient at baseline, 30 day and one year. Assist in data collection Establish follow-up for patient#s post procedure with structural heart clinic and cardiology Request records from outside providers for patient#s that have followed outside of SMC/MGC network Data entry into tracking system for continued patient follow-up (Excel spreadsheet) Call patient post procedure day 2#3 for readmission reduction. Liaison on for in-patient nursing units and cath lab staff. Liaison on for patients and families between departments Attendant heart team meetings Schedule and coordinate biweekly multidisciplinary conference Assist in resolving insurance denials Upkeep of patient medical records, tracking forms in summary Assist in filling out FMLA, insurance forms and other pertinent documents for patient Work with team regarding creation of patient education materials # Physical Requirements Ability to lift objects weighing up to 25 pounds.
